The Care Coordinator will provide guidance, education, and support to youth & young adults, as assigned to ensure goals related to education, vocation, living situation, well-being as well as other life skills are developed and met. The Care Coordinator will facilitate curriculums related to mental health resiliency to identified youth to increase their coping skills. The Care Coordinator will conduct outreach and assessments and assist clients with developing and fulfilling their case plans. They also help the client navigate health care and treatment systems, enroll in benefits programs, access appropriate and affordable housing, and connect with a support network. They provide case management services to youth and work with the Youth, Family, School Personnel and DFCS Case Manager to help clients meet holistic needs and overcome challenges. This position’s primary duties include but are not limited to the following:

  • Provide wraparound case management guided by comprehensive screening/assessment to treat SUD/AUD and COD
  • Conduct case management sessions to monitor and support clients’ progress
  • If applicable, collaborate with the Family to develop a family case plan
  • Develop and monitor Individualized Service Plans based on assessment of history of trauma, strengths, and needs
  • Display positive role-modeling to promote self-esteem and healthy boundaries
  • Make necessary partnerships to meet member's health, well-being, education, mental health, and employment and future planning needs (i.e. Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges and tech schools, etc.)
  • Research and/or attend trainings to educate self on best practices to provide adequate services to members
  • Bachelor’s degree in social work, counseling, psychology, public administration, or related social service/behavioral health
  • 3-5 years of experience in social services, youth services, and/or addiction and recovery services
  • Experience dealing with behavioral issues and people in recovery highly desired
  • Ability to effectively relate to consumers, parents/custodians of all social, economic, ethnic and educational backgrounds, with firmness, fairness and concern
  • Solid decision-making ability as related to the safety and well-being of consumers, families, community and staff
  • Be guided by precedent and established policy and procedure in decision-making
  • Dependability, emotional stability and physically capable.
  • Ability and/or experience in working with severely emotionally disturbed children, adolescents and their families
  • A valid driver’s license in the State of Georgia
  • Have TB test as required and fingerprints and drug screens in the office prior to employment
