The Medical Oncology Nurse Practitioner works collaboratively within a multidisciplinary health team and is responsible for providing and coordinating comprehensive care to both well and ill patients in the in-patient and out-patient setting. Intervenes in acute and/or chronic illness and coordinates care between in-patient and out-patient oncology services at Sibley Memorial Hospital and the Johns Hopkins Institution. Works with hospitalists, medical oncologists, nursing leadership, and Medical Oncology Director on in-patient oncology services serving as the clinical resource for admitted patients from the Kimmel Medical Oncology clinic. Monitors quality of oncology care across the in-patient-out-patient continuum. Obtains appropriate histories, performs physical examinations, orders diagnostic testing as well as develops care and treatment plans. Other activities are in support of patient/family education, staff education, unit activities, and psychosocial care of patients and families.
Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Patient Care:
1. Provides comprehensive, age appropriate and episodic care to patients as a member of the multi-disciplinary health care team specifically dedicated to oncology.
2. Collaborates with physicians, hospital and community providers, and all staff members.
3. Conducts comprehensive and/or episodic health assessments of patients/families with potential, acute, and/or chronic health problems.
4. Performs history and physical examinations as it relates to chief complaint, following guidelines of specific protocols. Discriminates between normal and abnormal findings in the history and physical.
5. Identifies differential diagnoses.
6. Assesses severity of illness and establishes plan for evaluation.
7. Records and documents assessment data necessary to maintain, coordinate, and/or expedite required services as well as billing.
8. Orders appropriate laboratory studies, radiographic studies, and other tests specific to the diagnosis or protocol. Makes referral to appropriate specialties. Writes chemotherapy orders following guidelines set forth by the Kimmel Cancer Center and AOCNP is obtained.
9. Follows daily and long-term progress of oncology patients including history and physical examinations, orders and interprets re-staging tests, and assesses status independently and or with consulting of physician partner. Establishes and implements plan of care based on those interpretations and observations.
10. May perform lumbar punctures, bone marrow aspirations, and biopsies along with other necessary procedures with appropriate privileging. (Special Procedures are designated as procedures that require special training or have institutional guidelines and criteria for such privilege. Institutional guidelines and criteria may be obtained from the department and section.
11. Prescribes medications including oral, parenteral, intramuscular, or subcutaneous drugs.
12. Prescribes controlled medications within the District of Columbia's advanced practice nurse scope of practice and per Drug Enforcement Administration (DEA) guidelines.
13. Follows up with patients and outside clinicians to ensure ongoing coordination of care.
14. Provides outside referring physicians with reports on patients' status by ordering, interpreting, collecting, and communicating results and recommendations. Consults with referring providers regarding questions specific to the treatment and/or its complications, suggests treatment plans, and makes appropriate referrals.
15. Provides education to patients and family members regarding diagnosis, complications, and treatments.
16. Participates openly and objectively in own performance evaluations, accepting constructive criticism, and sets realistic, measurable goals with manager/evaluator. Provides constructive input for co-worker performance appraisals to manager/evaluator as requested.
17. Demonstrates flexibility in scheduling to meet hospital, department, and patient care demands.
18. Participates in department projects and problem solving to ensure an efficient, safe environment for patients.
19. Participates on departmental projects and hospital committees/teams as assigned. Communicates information appropriately between unit and committee/team.
20. Assumes responsibility for own continuing education, planning for educational needs with the manager. Attends appropriate in-hospital and outside educational programs as assigned.
21. Understands and complies with all appropriate departmental, personnel, and hospital policies. Directs and advises others to follow all policies/procedures. Provides manager with input regarding the needs for policy/procedure changes as necessary.
22. Performs other duties as required.
Provides Emergency Care:
1. Maintains airway, breathing, and circulation of patient in crisis.
2. Starts and provides basic CPR.
3. Orders and administers anti-anaphylactic and supportive medications.
4. Maintains CPR certification.
Fiscal Responsibility:
1. Assumes fiscal responsibility with working knowledge of evidence based medical model.
2. Effectively uses resources of the patient and the institution.
3. Is aware of and adheres to specific capitations and limitations of the patient's insurance(s) while organizing and optimizing individualized care based upon the disease states and restraints imposed by the insurance provider(s).
4. Utilizes referral to outside home health agencies as well as palliative care and hospice providers to optimize patient care in the most appropriate setting.
5. Has working knowledge of and utilizes all available community resources, especially when the patient's resources may be limited or unavailable in order to provide the best medical care and social well-being
Scope of Responsibility:
1. Knows the formal and informal departmental goals, standards, policies, and procedures, which may include some familiarity with other departments within the division or hospital. Is sensitive to the interrelationship of both people and functions within the department.
2. Takes initiative to educate and enhance skills of other non-physician health care providers by planning in-services or other informal educational opportunities.

1. Arranges and organizes work to meet changing priorities. Assumes authority and responsibility for coordinating and making decisions regarding patient care. Assumes responsibility and accountability for organizing time and workload to meet patient, staff, and departmental needs.
Authority:
1. Assumes authority for coordinating and making decisions regarding patient care. May direct the activities of the non-physicians health care team regarding patient needs.
Reporting:
1. Reports to the attending physician in charge of patient's care and ultimately to the physician partner.
Qualifications
SKILLS AND ABILITIES REQUIRED:
1. Advanced clinical skills in specialty area or equivalent.
2. General computer knowledge and capability to use computer.
3. Ability to incorporate into practice theoretical concepts, advanced clinical and psychosocial skills with patients, families, and interdisciplinary health care team members.
4. Analytical ability necessary to diagnose patient's illnesses and to develop and implement plans of action for long and short-term management of acute and chronic health problems.
5. Advanced interpersonal skills necessary to communicate effectively with all members of the interdisciplinary team members to enhance collaboration and problem solving.
6. Ability to communicate effectively verbally and in writing, in order to provide information to patients and other health care personnel.
7. Ability to function effectively in a fluid, dynamic, and rapidly changing environment. Ability to adapt to changing assignments/priorities.
8. Must demonstrate knowledge of the growth and development across the life span and possess the ability to assess data reflective of the patient's requirements as it pertains to the patient's age-specific needs and to provide the care needed as described in the unit's policies and procedures.
9. Annual performance appraisals will measure the level of knowledge and skill maintained through the use of criteria-based job standards. Employees must maintain a satisfactory level of knowledge/skills to continue employment.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED:
1. Licensed to practice as Nurse Practitioner in Adult or Family Health in the District of Columbia to include controlled substance (CS) and DEA licensure.
2. Master's degree in Nursing from an AANP or ANCC accredited school.
3. Two years clinical experience as an oncology nurse practitioner preferred
4. Currently certified as a Nurse Practitioner (CRNP) by national credentialing institution and commitment to maintain certification as required.
5. AOCNP preferred.
6. Current CPR Certification required.
