Job Title: Registered Nurse - Long Term Care
Location: Oak Bluff, MA

Job Summary: We are seeking a compassionate and dedicated Registered Nurse to join our team. As a Registered Nurse in our Long Term Care facility, you will provide exceptional nursing care to residents with a focus on promoting their overall health and well-being. Your primary responsibility will be to deliver personalized care plans, administer medications, monitor vital signs, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. Responsibilities:
- Assess, plan, implement, and evaluate individualized care plans for residents in accordance with their physical, emotional, and psychosocial needs.
- Administer medications and treatments as prescribed, ensuring accuracy and adherence to established protocols.
- Monitor residents' health status, vital signs, and any changes in condition, promptly reporting any concerns to the appropriate healthcare professionals.
- Collaborate with physicians, nurse practitioners, and other healthcare team members to develop and modify care plans, ensuring continuity of care.
- Provide education and support to residents and their families, promoting self-care and empowerment.
- Maintain accurate and detailed documentation of residents' medical histories, treatments, and progress.
- Oversee and coordinate the work of nursing assistants and support staff, ensuring a high standard of care delivery.
- Promote infection control practices, maintain a safe and clean environment, and adhere to regulatory guidelines.
- Participate in interdisciplinary team meetings, quality improvement initiatives, and ongoing professional development activities.
Qualifications:
- Current and valid Registered Nurse (RN) license in MA.
- Bachelor's degree in Nursing (BSN) preferred, but an Associate Degree in Nursing (ADN) with relevant experience will be considered.
- Previous experience in long-term care, geriatrics, or a similar setting is strongly preferred.
- Proficient knowledge of nursing principles, practices, and regulations.
- Excellent clinical assessment and critical thinking skills.
- Strong communication and interpersonal skills to effectively interact with residents, families, and healthcare team members.
- Ability to prioritize tasks, manage time efficiently, and adapt to changing situations.
- Basic computer skills and familiarity with electronic medical records (EMR) systems.
