PrEP Navigator

The Pre-Exposure Prophylaxis (PrEP) Navigator provides intensive care coordination, support, and services to HIV negative individuals to prevent HIV and HIV positive individuals who are unaware of their status. They will be responsible for helping clients navigate and access HIV testing, PrEP, post exposure prophylaxis (PEP), care and treatment, and other community resources. This position will primarily focus on communities that may be vulnerable to HIV acquisition.


Core Duties:

  • Work with providers and support staff to identify potential PrEP clients and those in need of PrEP navigation services.
  • Provide support to HIV negative individuals who are interested in starting PrEP and those already utilizing PrEP care.
  • Complete screenings to identify readiness for PrEP and other barriers to care.
  • Assess benefits and insurance, and if needed, provide assistance to access payor programs.
  • Provide PrEP disease prevention and PrEP adherence education and counseling.
  • Effectively collaborate with all individuals involved in PrEP care and program delivery for the purposes of supporting high quality PrEP programming.
  • Make referrals for external services needed to support PrEP care and follow up to ensure service delivery.
  • Provide ongoing follow up to ensure PrEP adherence.
  • Serve as a fully functioning member of the Prevention and Community Engagement Team, able to participate in daily activities and functions to maintain overall operations.
  • Provide outreach to relevant communities with a focus on men who have sex with men of color and transgender persons.
  • Conduct rapid HIV and STI testing in clinic or during outreach events.
  • Provide education and risk-reduction assessment amongst HIV positive patients who are unaware of their status.

EDUCATION AND/OR EXPERIENCE AND OR LICENSE/CERTIFICATION

  • Bachelor's degree in public health OR two years' experience working in HIV/STD prevention or other community health programs required.
  • Familiarity with PrEP/PEP and other biomedical interventions.
  • Experience in a healthcare setting is preferred.
