The Lasata Senior Living Campus is owned and operated by Ozaukee County and offers a 130-bed care center that offers rehabilitation and skilled care nursing services. The campus also includes a 60-unit assisted living facility, and a 60-unit independent living apartment building.
Supervises and manages resident care
Coordinates all ongoing resident care with physician, resident, families, nursing staff and other Lasata campus departments. Attends resident care conferences. Makes daily rounds on all floors to validate staffing, assignments, and communication with nurses and aides of expectations and responsibilities. Accompanies physicians on rounds and helps coordinate on-going as well as new and special treatments. Participates in QAPI and other committees as assigned. Keeps abreast of professional developments by reading and by attending conferences and training sessions. Ensures compliance for interventions of “At-Risk” patients. Determine any follow-up from off going shift supervisor and assign follow up to unit nurses and validate compliance. Check each 24 hour/board and line list for communication changes to ensure appropriate residents are placed on board (falls, isolations, wounds, weight loss, etc.) Round each dining room at dinner and assure residents are in the dining rooms and assisted; in addition, at risk or weight loss patients are being fed and assisted. Ensure labs and medical orders are being follow up on. Assure showers/baths/skin assessments are completed and documented on each unit. Assure and validate that wound residents are being repositioned and toileted. Communicate and report with clinical management team issues and concerns in a timely fashion.
Supervises nursing personnel
Evaluates the performance of nursing personnel and acts as required. Works closely with In-Service Director in regard to procedures and training of aides on assigned shift. Actively participate in corrective actions that are being implemented to assure all staff on yoru shift are providing quality care and following standards of nursing practice. Follow up with education and/or other assignments from the Director of Nursing as needed.
Admissions
Introduce and welcome new residents and their family. Work with unit nurse to tag team needs for completion/clarification of orders/48 hour care plan/skin assessment completed within a 2 hour window. Assist unit nurse as needed.
Starting hourly compensation varies based on experience.
