If you have education and experience being qualified and registered with the Board of Counseling in accordance with 18VAC115-80 as a QMHP-A or QMHP-C (Trainee still applies), and a minimum of one year of experience providing direct services to adults with various mental health challenges. Our Mental Health Skill Builder is responsible for providing direct services to referred individuals in their homes and within the community according to their ISP.
Responsibilities:
- Provide individual skill building services to eligible individuals
- Promote the program in a positive manner to elicit referrals and maintain census and growth of the program
- Maintain an accurate schedule community sessions to be reviewed at any time
- Submit a copy of all Restraint Forms and Incident Reports to your direct supervisor
- Ensure that every Incident Report is completed immediately after the incident and submitted to the direct supervisor or Program Coordinator the same day
- Complete all Progress Notes using sound clinical documentation and abiding by all agency, Medicaid, and licensure standards
- Constantly monitor and document individuals’ progress towards completion of treatment goals and objectives
- Attend all annual mandatory agency training (Handle With Care, CPR & First Aid, Human Rights, HIPAA etc.)
- Identify service areas that require strengthening and seek out opportunities to attend training in such areas
- Other duties as assigned
