Creative Lifestyles is looking for Medical Coordinators in St. Clair Shores (near 11 Mile Rd. and Little Mack) AND Harrison Township (near I-94 and Metro Parkway). These employees plays a vital role in advancing our mission to make a difference in the lives of our residents. St. Clair Shores is a 6-bed adult foster care co-ed home and Harrison Township is a 16-bed AFC co-ed home.

The shifts for this position vary between morning shifts and split shifts (partial day/partial afternoon) with changes to working hours based on the needs of the facility. The hourly rate is $16.25 plus $2.35 Premium Pay (total of $18.60 per hour)- higher potential for those with experience and current valid training. Overtime is required when needed (overtime rate= $26.725).

This role is ideal for someone who enjoys being a leader with an increased level of responsibility, is comfortable working alone and with a team, and realizes that frequent communication is the key to success!

Duties include:

• Daily communication with and following directives from the facility’s Supervisor
• Scheduling, attending, and overseeing medical appointments for the residents
• Medications- records, orders, and organization
• Correspondence with Doctors and Nurses
• Communication with Direct Care Staff
• On-call 24/7 for all medical needs of the residents
• Direct Care work including medication passing, meal preparation, and light housekeeping
• Scheduling regular activities for the residents
• Transporting residents to and from work sites using a company provided van
• Recording and tracking resident leaves of absence
• Ensuring facility's vehicle is well maintained and serviced according to the manufacturer's recommendations
• Reviewing and completing a home maintenance checklist on a monthly basis
• Afternoon, night and weekend shifts will be required when covering staffing shortages, absences, or scheduled vacations
• Online Training will be provided if not current with Macomb County's training requirements
• Must have a good driving record and valid Michigan driver’s license
