Job Compensation
$70,000-$75,000 based on education, experience, and certifications
Your Impact Here at MCC
- Responsible for collaborating with both professionals, therapists, and families in order to ensure a smooth transition for students from school into adult services
- Providing workshops to families regarding enrollment in, and services provided by OPWDD
- Individual assistant with families to enroll in OPWDD services including, but not limited to: acquisition of outside agency services, crisis support services, guardianship, special needs trusts, SSI, respite services, and post-secondary placements
- Maintaining an ongoing familiarity with NYS & City adult services – specifically day habilitation and job coaching agencies, as well as residential programs.
- Perform other duties as assigned by supervisor
Requirements
- Master’s Degree and Valid LMSW or LCSW license to practice in New York State
- Experience working with young adults with disabilities, adult services, and/or transition planning preferred.
- Energetic, can-do attitude and a willingness to collaborate with teachers, related service providers, parents, and other faculty.
