Our client, a leading health plan with contracted work in Suffolk County on Long Island, has engaged us to find a strong Social Worker with knowledge of geriatric and behavioral social work, to join their team of professionals on a full time basis.
As social worker, you will work collaboratively with an interdisciplinary care team to identify and meet the psychosocial needs of the members, provide support to the members and their caregivers, and provide key clinical support to the facility staff.

Position Qualifications:
- Excellent communication skills, with the ability to work with people from diverse backgrounds and experiences
- Masters in Social Work, LMSW or LCSW is required
- Two years of prior experience in the healthcare field a plus
- Motivates and engages both the members, ISNP staff and caregivers
- Demonstrates the ability to work independently
- Access to reliable transportation and willing to travel
- Experience working with the psychiatric population desired
- Fully vaccinated against COVID-19
