What We Do

The Care Coordination Program provides services as designed by the NYS DOH Health Home Model, based on unified systems of care to coordinate and integrate physical and behavioral health care, substance use treatment, and social services. The goal of Care Coordination is to minimize the use of hospitals and emergency rooms by providing access to all of the services an individual needs to assure he or she stays healthy.

Job Summary

The Intake Specialist is responsible for the initial outreach and enrollment of clients for the agency through provider relationships and individual engagement. The successful Intake specialist will achieve a high rate of successful enrollments. You will also:

  • Conduct aggressive outreach in the field and via phone to prospective Health Home clients as well as complete follow up, as needed.
  • Conduct outreach and provide clients and internal or external providers with an overview of New Horizons Counseling Center Health Home model.
  • Complete client detail intake pre-screenings, Medicaid eligibility, explanation of Health Home and achieve daily and weekly quota.
  • Assist client with non-active Medicaid through initial referral and entitlement process
  • Achieve a conversion rate comparable or above industry standard
  • Participate in staff meetings and trainings
  • Consistently exhibit core values of team work, empathy, flexibility, accountability and empowerment.
  • Perform other duties as requested.

What You Need

High School Diploma Required

Skills and/or Experience Required:

  • One (1) year of sales/marketing and/or outreach experience required with the ability to connect with and engage the target client population.
  • Friendly, enthusiastic and engaging demeanor with the ability to market and explain the program to potential enrollees.
  • Sensitivity to chronic conditions, substance use, mental illness and other related issues
  • Have the ability to maintain filing system and electronic documentation.
  • Must be able and willing to conduct field work and navigate public transportation.
  • Sales/Marketing experience is required with demonstrated capability to perform under pressure and reach monthly quotas
