Intake Nurse (RN/LPN)
Rocky Mountain Care is looking for a full-time nurse to join our Central Admissions department. This is an office position with no direct patient care.
Location- Woods Cross Office

Requirements:
- Current license as a licensed LPN/RN from the Utah State Board of Nursing
- Minimum 1-year experience in general medicine or related field
- Skilled nursing facility experience preferred
- Good verbal and written communication skills
- Basic computer knowledge
- Ability to effectively problem solve
- Provide excellent customer service
- Knowledge of Medicare/Medicaid home care and hospice criteria preferred
- Knowledge of Medicare, Medicaid and 10A criteria
Responsibilities
- Receive information from referral sources and coordinate care for patients
- Verify and enter physician orders
- Identify level of care needed
- Receive and give verbal orders accurately
- Review clinical documentation to determine Skilled Nursing placement
- Exceptional interpersonal and customer service skills
- Ability to communicate clearly with pharmacist
- Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
- Manage after-hours and weekend referrals (rotating on call)
Schedule
- Monday – Friday (rotating shifts: 8:30-5:00pm / 10:00-6pm)
- One schedule weekend every 8-12 weeks, and rotating monthly on-call
Job Type: Full-time
Pay: $28.00 - $30.00 per hour
