Job Description

The IDD Intake/Admissions Coordinator provides services to children and adults with Autism, Developmental or Intellectual disabilities based on the Person-Centered Thinking model through assessment of needs, development of service plans, facilitating service linkages and monitoring for their effectiveness, provision of crisis intervention/prevention supports and the delivery of advocacy services. The ability to work with families and sensitivity to various agency and interagency personnel to accomplish objectives. The ability to work with families and sensitivity to various cultural differences is critical. The purpose for the position is to empower individuals served to achieve life goals and to live effectively within their families and/or community setting. IDD Intake/Admissions Coordinator will work with individuals who are seeking IDD Services and/or Autism Services. This includes but is not limited to advocating and linking individuals interested in job readiness and/or employment activities through the Employment First project. The coordinator will participate in interagency planning and CRCG meetings to assist in bridging gaps in services and avoid duplication in services in the communities where individuals receive services.

The IDD Intake/Admissions Coordinator requires frequent use of initiative and independent judgment; assertive, professional intervention; frequent in person, long-term contact with individuals, families and service providers and the ability to work with minimal supervision. Job responsibilities may require traveling within the service area and flexible hours and schedule. Essential duties include completion of comprehensive assessments of individual needs and desires, service planning, referral and linkage, monitoring and advocating on behalf of individuals served, documentation of services provided and interaction with the individual/family served.

IDD Intake/Admissions Coordinator duties relate to completing eligibility screenings for various funding streams (ICF/ID, CFC, Autism and GR) from multiple referring agencies and individuals, referrals to resources, admissions to GR, referrals to ICF/ID and CFC services, Autism services and crisis intake support and intervention. This position may be trained in multiple aspects of the job to allow for backup support within the unit and agency. This job will also be back up support for initial intakes, Interest List calls for HCS and TXHML as needed, support for CFC Initial ID/RC’s and Single-Need Authorization plan renewals as needed

Salary: $20.44 – $27.19/hr

Credentials
Minimum Qualifications: A bachelor’s degree or higher with a major in social, behavioral, or human service field including, but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, or criminal justice.

Must have a valid Texas Driver’s License and maintain approved driver’s status throughout employment.

Preferred Qualifications: This position requires a bachelor’s degree in a human related field; at least one year case management or service coordination experience working with individuals diagnosed with Intellectual or Developmental Disability is preferred. Knowledge of appropriate community resources and experience with Medicaid Waiver programs is ideal. Local travel will be required to meet with individuals and their families in the community. Occasional out of town travel may be needed to assist in service linkages. Must be able to maintain a driving record acceptable to the agency’s insurance carrier. Flexibility is needed in the work schedule to accommodate the needs of individuals and/or their families. Interaction with others is necessary on a daily basis. This position requires a demonstrated ability to use good clinical judgment, get along well with others and work effectively in a fast-paced setting. Computer skills and documentation skills are utilized on a regular basis in the performance of assigned duties. Qualified Mental Health Professional (QMHP) credentials are also preferred.
