Purpose:

Under the direct supervision of the HRT Director this position provides direct care services to homeless and at-risk individuals and families living in scattered site Non-Congregant Shelter locations.


Duties, Functions and Responsibilities:

  • Place clients who have been referred for housing into an appropriate Non-Congregant Shelter location.
  • Conduct intake and assessment procedures as required to ensure client eligibility.
  • Conduct regular well-checks (unit visits) on residents, to ensure health and safety.
  • Conduct regular Oversight to ensure that each client receives assistance completing SNAP, Medicaid, SOAR, and/or other applications, and to address any other social barriers the client faces i.e. Birth Certificates, NV ID, Social Security Cards.
  • Enforce all HELP rules and standards.
  • Continuously monitor clients' progress and provide follow-up and wrap around services as needed.
  • Monitor and report all program violations within 24 hours of the incident.
  • Provides crisis intervention as needed.
  • Completes all required paperwork, including incident reports, damage reports, and if necessary, police reports.
  • Provides transportation assistance i.e. bus passes/ transport to appointments when necessary.
  • Assist with any supply distributions.
  • Builds positive rapport with residents, and verbally de-escalates problematic behaviors.
  • Work with staff (case managers) from participating programs to assist in the transition of the clients to referred housing programs. To ensure a successful transition into the clients new housing program.
  • Complete resident discharge procedures (pack up belongings and clean out units) as required.
  • Enter case notes and data into HMIS daily in real time data, reflecting activities of the clients' progress, and always maintain audit-ready files.
  • Other duties as assigned by supervisor.

Knowledge, Skills and Abilities:

  • Knowledge of local social service agencies and programs as they pertain to homeless and at risk populations.
  • Knowledge of applicable statutes, ordinances and codes.
  • Experience and understanding of homeless at risk population issues.
  • Communicate effectively with persons from a variety of social, cultural and sensitivity to culturally diverse populations.
  • Effective written, verbal and listening skills. Independently initiates and responds to correspondence.
  • Ability to maintain order and exercise appropriate judgment in crisis situations.
  • Is flexible and can adjust to shifting priorities.
  • Establish and maintain cooperative working relationships with residents, follow employees, other agencies and the general public.
  • Organize work schedule and budget time.
  • Understand and follow all agency policies and procedures.
  • General computer knowledge and ability to use Microsoft Office software.
  • Ability to work flexible hours as required by resident case load.


Qualifications, Education and/or Equivalent Experience:

  • High School Diploma or GED
  • 2 years of experience working with homeless or at risk populations, preferably in a shelter or residential facility.
  • Must have and maintain 1st Aid/CPR certification.
  • Must pass a background check and pre-employment drug test. Random drug screening is performed.
  • Must have a valid Nevada driver's license and clean DMV record. Must have five years driving experience. Driver History Reports are required to be submitted on a quarterly basis.


Working Conditions:

Work is performed in the community, and in an office environment. Work is performed by well checks, transporting clients to/from appointments and outreach efforts. May be subject to sitting, standing, squatting, walking, bending, lifting, pushing and pulling, and simple grasping. Outreach is performed in areas such as tunnels (storm drains), and desert conditions (dirt fields), regardless of weather conditions.
