The mission of Alum Rock Counseling Center (ARCC) is to heal families and inspire youth to reach their full potential. For 48 years, ARCC has provided a wide range of behavioral health counseling and school-based support, designed to promote health equity & educational equity, and ensure that high-risk youth stay safe, in school, and drug & violence free. The agency supports a predominantly Latinx (70%) population in underserved schools and neighborhoods throughout Santa Clara County with a focus on the high-need area of East and Central San Jose. ARCC staff provide services to youth and caregivers who otherwise might struggle to afford and/or access quality care. In 2020-21, the agency served 6,000+ individuals, through 13 different programs, including individual and family counseling, substance use prevention education, youth mentoring, family reunification, parent training/support, and services in 40+ high-need schools.

POSITION SUMMARY:
The First 5 Home Visitor provides culturally competent, strength-based home visitation services to families with children ages 0-5. Responsibilities include assessment of child/family needs, treatment planning, case management, and parenting skills development. Services are provided at the clients’ homes, school sites, the community and in-clinic.
DUTIES
- Conduct developmentally appropriate screening and assessment of children’s needs, strengths, and promote healthy child/family development practices.
- Formulate, implement and monitor individual child/family development service plans.
- Conduct child observations and make appropriate referrals for specialized services.
- Facilitate parent-child interactions through effective home-based strategies.
- Provide coaching and guidance in the areas of family management practices.
- Conduct home visits and provide behavioral supportive services to children and families.
- Collaborate with clients and link families to First 5 service providers.
- Conduct follow-ups and monitor service delivery and client progress.
- Maintain confidential client records and files and complete required paperwork including treatment plans, psychosocial histories, and progress notes.
- Document all chart contacts within the time-frame established by the agency.
- Maintain a caseload of 14-16 clients per week and meet productivity established by agency.
- Administer county evaluation surveys as required by contract.
- Understand and adhere to the laws governing child abuse and elder reporting.
- Attend meetings and trainings necessary for professional development and as required by the agency or its funding sources.**
- Provide Triple P curriculum individually to families or within a group setting.
- achelor's Degree in counseling, psychology, child development or related field.
- One year of experience working with children ages 0-5 and their families in a community-based setting or are currently enrolled in a mental health related Master’s Program.
- Experience working with low-income and different ethnic populations. Understanding of and sensitivity to economic and cultural issues in diverse communities.
- Bilingual/biliterate competence in Spanish/English required or willing to use a translator to provide services.
- Effective verbal and written communication skills, interpersonal and collaboration skills, as well as computer literacy.
- Experience working in a Medi-Cal driven environment and completing Medi-Cal documentation preferred.
- Able to work independently and function as a positive member of a team. Able to meet deadlines.
- Valid CA driver’s license, an automobile for agency travel, and proof of current auto insurance.
- DOJ fingerprint and background clearance, and negative TB clearance required
- Must be fully vaccinated and boosted
