Job Responsibilities

• Assist clients in their homes with daily living activities including personal care and grooming, bathing, dressing, toileting, hair care, and skincare
• Assist patients with walking, transferring in and out of wheelchairs, and changing positions in bed
• STNA/Home Health Aide provides patients with companionship and emotional support
• Provides assistance with needs around the home
• Document services provided in the client’s electronic medical chart
• Work in cooperation with our caring home health and hospice team
• Adhere to professional standards, follow policies and procedures and abide by federal, state, and agency requirements as well as quality accreditation standards

Qualifications and Skills

• High school diploma or equivalent
• Physically capable to lift and transfer patients
• Excellent communication skills, including the ability to communicate sensitive information with empathy (“bedside manner”)
• Ability to work as part of a team and handle multiple tasks safely and effectively
• Reliable transportation, current driver's license, and insurance
