PURPOSE: To develop and coordinate individual programs, community and volunteer activities. Provide training to staff to ensure knowledge and skill in working with individuals served. Provide direct care and training to individuals served.

ESSENTIAL FUNCTIONS:

  • Provide Adult Day supports as needed pursuant to individual’s plan (minimum of 15 hours per week).
  • Responsible for daily programming and staff oversight, ensuring the health and safety of individuals and staff.
  • Coordinate and participate in the development and implementation of individual plans in coordination with other members of the individual’s team.
  • Assist in coordinating the intake function: Screen telephone inquiries for potential clients, conduct potential client interviews and distribute marketing materials, begin new client processing. Provide orientation for new clients, families and guardians.
  • Plan and implement in-service and other staff development activities as appropriate.
  • Establish procedures to monitor capacity, availability and initiate placement from waitlist as needed.
  • Establish procedures to coordinate billing and fee collection information with the fiscal office.
  • Ensure all referrals, wait list and information calls for the program are documented.
  • Ensure all client records are current and complete as required by license agents and organizational policies.
  • Utilize a variety of professional assessment procedures and tools to determine skill level, interests, abilities, and needs. Prepare reports, evaluations, and letters reflecting individual’s progress and concerns on a daily, quarterly, or yearly basis as needed and/ or required.
  • Develop, monitor and schedule community activities, volunteer opportunities, activities of daily living, crafts, etc., to maximally integrate each individual.
  • Provide training, assistance and intervention as needed to individuals served per the DODD regulations.
  • Maintain client confidentiality as required by County DD standards and in compliance with HIPAA regulations.
  • Prepare and review individual’s documentation for compliance /regulatory standards.
  • Responsible for supervision of staff to include: Implement evaluation procedures for staff in accordance with established procedures.
  • Be responsible for hiring, supervising, training and terminating staff in accordance with Easter Seals, OSHA, FLSA and other applicable labor requirements.
  • Provide support to program staff to ensure quality service as well as a safe environment.
  • Ensure staff maintains required certification and documentation to meet state and local requirements for employment.
  • Oversee administration of direct services to clients enrolled in program.
  • Responsible for scheduling of staff to meet the needs of program.
  • Maintain MUI/UI log.
  • Prepare monthly and quarterly reports.
  • Assist in ensuring compliance with all appropriate standards including CARF and DODD.
  • Act as a liaison with families, caregivers, advocates, and other stake- holders.
  • Actively participate in the recruitment, daily activities, and retention of individuals attending program.
  • Order and maintain program related supplies within budgetary guidelines.
  • Ensure the program area is clean and routine maintenance is completed.
  • Plan and participate in all monthly community evening activities.
  • Maintain Outlook Calendar.
  • Maintain a clean and organized work area.
  • Perform other duties as assigned by Community Services Manager.

EXPERIENCES:

  • Minimum of 5 years’ experience working with individuals with disabilities, at least 2 years supervisory experience
  • Current CPR/First Aid
  • Bachelor’s degree preferred.
  • Valid Ohio Driver’s License – with less than 6 points

SKILLS & ATTRIBUTES:

  • Strong organizational and leadership skills
  • Excellent written and oral communication skills.
  • Ability to prioritize multiple tasks.
  • Working knowledge of multiple activities planning process
  • Experience in planning community-based activities
  • Knowledge of MS Word, MS Excel and other needed computer programs necessary for program documentation
  • Ability to increase knowledge and skills through on going training
  • Ability to change directions as required by the organization
  • Self-Motivated

PHYSICAL REQUIREMENTS:

  • Must be able to lift, carry push or pull a minimum of 50 pounds.
  • Frequently requires sitting, standing, walking, bending, squatting, kneeling and reaching.
  • May be required to be on feet hours at a time.
