JOB SUMMARY:

The prospective consultant will assist with the implementation of Grant Funded Prize-Based Contingency Management, (PBCM) Model to reinforce positive behaviors in patients related to substance abstinence, which in turn will produce long-term positive change/outcomes for the term of the initiative from Nov 14, 2022 to March 13, 2023. Contingency Management is an evidenced-based reinforcement program component that will be applied to a sub-set of patients with positive toxicology screens and non-compliance with group therapy attendance enrolled in Grace Medical Center New Hope Treatments Medication Assisted Treatment Program struggling with abstinence from alcohol and other illicit substances.

• Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.
• Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
• Vendor shall work collaboratively with Grace Medical Center’s New Hope Treatment Center’s leadership to provide (i) Contingency Management Initiative outcome findings; (ii) ongoing engagement with patient participants to complete pertinent documents that track patient success; (iii) review data analysis to identify key opportunities to address barriers and/or challenges that impede behavioral changes from being realized; and (iv) analyze patient outcomes with medical and clinical teams to identify key opportunities for Service Line improvement.
• Collaborate with Addiction Counselors to identify potential initiative participants.
• Maintain confidentiality of Patient Protected Health Information, (PHI) as mandated in the Health Insurance Portability and Accountability Act, (HIPAA) and 42 CFR Part 2, confidentiality of substance use disorder patient records.
• Review toxicology and treatment services non-compliance reports of potential patient participants.
• Conduct weekly one-to-one meetings with patient participants to complete a personalized Prize Based Contingency Management Worksheet to track the following: start date, behavior change/outcome, reward type, date reward given, and disposition/additional comments.
• Review the PBCM worksheet weekly with patient participants to reinforce positive behavior changes and discuss obstacles or barriers to achieving success.
• Meet weekly with assigned counselors via in person or on Zoom to discuss patient progress.
• Maintain the purchase and distribution of rewards, tracking outcomes, overall data collection, sharing outcomes with treatment team members comprised of: medical, clinical, administrative, and Peer Recovery Supports.
• Distribute rewards for positive behaviors through several reward types: (small, medium, and large) for consecutive toxicology screens and consecutive group attendance with reward incentives.
• Performs other duties as required.
