CHA is supporting health in a new way. Instead of just treating illness, we are helping entire populations of patients be as healthy as possible. To do this, we have built a system to 1) identify medical needs, 2) provide proactive care and 3) address hidden factors that affect health (ie. social determinants). For example: Do people have a safe place to live? Food to eat? Can they get to the doctor? Do they have insurance? People who need more support get a high-touch approach - coordinated care, appropriate education and direct linkages to local community programs. Our goal is to give every patient the right care at the right time. This lets CHA manage health care costs while helping people live their healthiest lives.

Summary:
Description
- Assist the Youth Wellness Coordinator (YWC) with outreach efforts to support youth
- Help build a coalition and capacity for positive change in the community
- Develop and maintain relationships with community partners
- Act as a liaison between community members and CPHD
- Working with other organizations to build partnerships
- Managing communication with community partners
- Assisting with scheduling outreach meetings with community partners
- Participating in meetings with various community partners
- Assisting with organizing community events
- Assisting the YWC with community assessments, such as:
- Community Readiness - the willingness of a community to address substance
- Resources and Assets - anything a community can use to create and maintain

Requirements
- Familiarity of Cambridge programs
- Interested in Public Health, community organizing, youth work/positive youth
- Excellent communication and organizing skills
- Weekly supervision
- Occasional evening or weekend work (to attend events)
- Professional development
- Opportunities to attend trainings
