Applewood Centers is a private, nonprofit agency providing mental health services, social services and education to children, adolescents and families living in Cuyahoga and Lorain counties. Dedicated to addressing a variety of behavioral, emotional and learning problems, we provide a continuum of care from outpatient counseling to residential treatment.

Position Summary: We are hiring an Intake Specialist to ensure the appropriate placement of clients throughout our residential treatment services; or referral to services of other agencies if appropriate. As the Intake and Admissions Specialist, you will be the primary contact for Crisis Stabilization referrals for residential treatment. This position works as part of the Clinical Services Team to also ensure all residential referrals (crisis and traditional) are assessed for placement, tracked and facilitated. This position may also include providing Case Management services for crisis clients and providing group services as needed. This position requires the candidate to provide 24/7 on-call services for crisis referrals 3 weeks per month. This position may also play an active role in meetings and trainings regarding the clients in the program.

Responsibilities Include:

• Complete preliminary clinical interview, assessments and initial (admission) treatment plan. Ensure that admission/intake paperwork is completed in accordance with government regulations and Agency policies. Schedule and facilitate admissions to residential treatment program.
• Responsible for informing and updating program managers, supervisors, and director regarding the status of admissions, waitlist, psych testing referrals, and information gathered from referral source as well as relevant intake interview.
• Responsible for tracking referrals to residential treatment program. Manages/tracks pending discharges and residential treatment department's "waitlist". Actively submits and tracks referrals for psychological testing.
• Conduct other residential specific trainings as indicated.

Qualifications:

• Master’s Degree in Social Work or related field.
• Valid Ohio LSW, LPC.
• Strong clinical skills including expertise in systemic family therapy, crisis intervention, family education, behavioral interventions and substance abuse therapy. Sensitivity in relating to persons of varying backgrounds and demonstrate ability to work with diverse groups of people possessing various strengths, aptitudes and abilities.
• Ability to perform job responsibilities with a high degree of initiative and independent judgment.
• Demonstrated oral and written communication skills and effective interpersonal skills. Practical and creative problem solving skills
• Proficiency in using Agency computer systems and software as required to perform essential job functions.
• A valid driver's license with approved driving record(less than 6 points), personal transportation and insurance, if required to drive on behalf of the agency.
