As the Client Relations Coordinator you will collaborate with physicians, nursing homes, assisted living facilities, extended care facilities, and home care personnel in the process of providing coordinated quality home care services for patients referred to CHI Health at Home.

  • Following referral of a patient to CHI Health at Home, assists social workers and/or care coordinators in coordinating quality home care for clients,
  • Determines possibility of patient appropriateness for home care.
  • Effectively communicates with appropriate disciplines involved in the care of potential home care clients, and provides information and recommendations to Home Care/Hospice staff.
  • Develops professional working relationships with referral sources, generating an open flow of information and support of home care goals; provides value added counsel, teaching and resourceful problem solving.
  • Available to all discharge planners, physicians, other facility personnel and clients to analyze eligibility and explain Medicare and Medicaid guidelines, insurance plan benefits, financing options, Home Care/Hospice policies, etc.
  • After referral, communicates with attending physicians to obtain verbal or written orders for home care services.
  • Assists in ordering equipment, oxygen, IV's, etc., as required to provide proper home care for the patient referred to CHI Health at Home.
  • After referral, determines individual patient needs for services, referrals, training and provides patient instructions on home care equipment and/or procedures, etc.
  • When requested, participates in the education of facility personnel on home care regulations, the variety of services available and trends.
  • When requested, participates in educating/in servicing new facility personnel on home care and the importance of the communication of this information to their clients.
  • Meets with the Director, Operations and other Home Care/Hospice associates to ensure continuity of care.

A compensation range is $48,000 to $71,000 the reasonable estimate that CHI Health at Home in good faith believes it might pay for this particular job based on the circumstances at the time of posting. CHI Health at Home may ultimately pay more or less than the posted range as permitted by law.

Qualifications

  • Graduate of an accredited school of nursing. Current Registered Nurse (RN) or LPN license to practice in the state(s) serviced.
  • Minimum of two years of home care experience.
  • Knowledge of home care regulations.
  • Strong critical thinking, judgment and decision making skills.
  • Strong Sales knowledge, skills and abilities
  • Excellent customer service, communication, and interpersonal skills with diverse client and staff.
  • Priority setting, time management and organizational skills.
  • Excellent communication skills, both verbal and written.
  • Excellent training and persuasive abilities.
  • Valid Driver's/Chauffeurs license and auto liability insurance coverage according to organizational policy is required.
  • Compliant with appropriate state certification/registration and organizational requirements as necessary
