We are seeking an entry level Discharge & Aftercare Coordinator to join our team in Winter Park, FL!

This position is responsible for:
- coordination of patient after-care plan, working with on-site case managers and / or coordination of appropriate referrals unable to be placed at our centers.
- Will identify and coordinate appropriate referrals based on clinical needs and insurance restrictions of patient which may include calling, scheduling, and follow-up confirmations.
- May also include calls to referral sources to determine appropriate needs for future patients and identify more appropriate referral accounts based on geography and type of clinical needs
Core Job Duties:
- Effective communication with on-site case managers
- Appropriate placement to company approved after-care locations
- Documentation in CRM software for patient record keeping
- Nurturing of business relationships through discharge planning
- Effective cross-departmental communication and coordination
- Outbound placement for inquiries unable to be treated at an ARS facility
Qualifications:
Qualifications:
- BA/BS or equivalent working experience
- Ability to use a computer
- Strong attention to detail
- Strong organization skills
- Outstanding communication skills
- Strong multi-task ability
- Customer focused and solution-oriented
- CRM and Microsoft Office Suite familiarity a plus
- 1-2 years in mental health, addiction or healthcare environment a plus
- Entry Level candidates and recent graduates are encouraged to apply
