We are seeking a Blended Case Manager to join our team in our(Delaware County) Sharon Hill, PAlocation.
Position Details
The Blended Case Manager will provide resource coordination, recovery services, and symptom management using a strengths-based model of case management to 20-25 Adults living with a mental illness. The BCM will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence, and chart documentation.
(75% community based / 25% program office)
Earn $20.98/hr or $22.98/hr with enhanced pay option.

Requirements:Our ideal candidate will possess the following skills:
- BA/BS in Psychology, Sociology, or related field or
- A registered Nurse or
- AS in Psychology, Sociology, or related field plus 1 year-experience working with the mental health population or
- 12+ credits in Psychology, Sociology, or related field plus 2 years-experience working with the mental health population, with 1 year of direct care contact or
- A high school diploma and 5 years mental health direct care experience in public or private with employment as a Case Manager prior to April 1, 1989
- Valid driver's license, with a clean driving record, and own a vehicle
- Travel within service area required(75% community based / 25% program office)
