Behavioral Health Care Specialist (Full Time) Job at System Health Solutions
Overview
This role is responsible for tracking and managing patient registry and progress, tracking time devoted to behavioral health care provided, resource and referral management, support for integrated behavioral health in person and via phone, support patient staffing meetings, and related duties by working collaboratively with the integrated behavioral health team to achieve desired clinical service and cost outcomes. Interacts directly with patients to support interventions initiated by the behavioral health and primary care teams, assisting in screening and treatment response measurements, providing information on internal and external resources, and coordinating referrals for sites not physically staffed by the integrated care service. Also responsible for maintaining a registry of all patients interacting with the behavioral health team and tracking time spent on the care of each patient. Identify and triage crisis needs.
Requirements:- High School Diploma or GED is required.
- Associates degree in a medical or behavioral health field OR a certified Medical Assistant is required.
- BLS Certificate is required.
- Requires strong organizational skills.
- Requires the ability to problem solve and work independently.
- Requires proficiency with EMR programs.
- Medical Office or Behavioral Health experience is a plus.
- Microsoft Office experience is a plus.
