Responsibilities
- Screen all Albright LIFE participants for possible services; evaluate and treat all participants who are accepted and could benefit from intervention as referred by a state approved referral source, establishing realistic treatment goals for participants who need OT; optimize the functional abilities and skills of participants in the Albright LIFE program; provide quality care as well as reasonable goals and outcomes within established guidelines.
- Participate as a member of the intake assessment team; complete home care Assessments with the social worker, to determine prospective participant’s clinical level of care; report determination by the assessment team to the intake coordinator and the interdisciplinary team.
- Embrace the team concept and interdisciplinary care in the LIFE Program environment. Participate as an active member of the interdisciplinary team, determining the OT goals of the care plan; monitoring the participant’s progress toward goals; documenting in the medical record in accordance with LIFE procedures; and, working closely with the Director of Health Services and Director of Service Excellence to monitor key benchmarks for OT services.
- Provide direct supervision of Certified Occupational Therapy Assistant’s (COTA’s) and Rehab Aides in tasks during group and individual treatment in a manner that promotes appropriate utilization of resources and facilitates continued developments of clinical skills in accordance with Commonwealth Practice Acts.
- Embrace the team concept and interdisciplinary care in the LIFE Program. Participate as an active member of the interdisciplinary team, determining the recreational activity goals of the care plan; monitoring the participant’s progress toward goals; documenting in the medical record in accordance with LIFE procedures; and, working closely with the Center Manager and Director of Service Excellence to monitor key benchmarks for recreational activities services.
- Serving as a member of the Interdisciplinary team, evaluates the participants’ educational, emotional and vocational needs; plans, organizes and directs activities in accordance with the participants’ needs, capabilities, and interests, per the treatment plan developed by the interdisciplinary team.
- Builds within the Day Health Center a sense of community and a warm social atmosphere in which participants are able to identify a support system; develops and implements new recreation programs and coordinates with various community resources to add diversity and enrich the recreational program.
- Supervises the Recreation/Activity Assistants within the department; Assists in the orientation and supervision of volunteers assigned to the Activities Department.
- Provides mobility and therapy assistance; wheelchair and van transfers; and complete range of motion and other restorative exercises
- Acts as Receptionist when needed. Greets and directs participants, visitors and staff. Answers and accurately directs incoming telephone calls. Operates all office equipment.
Requirements
- Valid COTA License
- Must have a current and valid Pennsylvania driver’s license and a good driving record
Hours
- Full time - 40 hours/week
- Shifts vary between day and early evening
