Assisted is an exciting place to be right now as we are experiencing tremendous growth. We are looking for a Spiritual Counselor on a per diem basis for our Hospice Division to meet with patients and families in the Los Angeles area.

- Assess the spiritual status of the patient, family, and caregiver.
- Provide meditation, counseling, prayer, sacred rituals or practices, active listening, and supportive presence, as appropriate.
- Document the spiritual assessment, goals for spiritual care and services provided, and the patient’s / family’s response to the spiritual care.
- Assist with funerals and memorial services as requested by patient and/or family.
- Communicate with and support the involvement of local clergy and/or spiritual counselors as possible and as desired by the patient, family and caregiver.
- Consult with and provide education to interdisciplinary team members, patients / families about spirituality related care and service.
