POSITION SUMMARY:


Under the leadership of Spiritual Care Coordinator, the Spiritual Care Provider is a member of the Interdisciplinary Team (IDT) responsible for providing spiritual care and bereavement services for participants, caregivers, and families.

SPECIFIC DUTIES AND FUNCTIONS:


  • Demonstrate the knowledge and skills necessary to provide spiritual care to participants and document services rendered. Also, communicate effectively with the Interdisciplinary Team, staff, caregivers, and family.
  • Conduct Spiritual Assessments and determine the spiritual needs of the participant and develop a Spiritual Care Plan to address those needs.
  • Provide spiritual care to participants via phone call, direct contact, Spiritual Wellness Sessions, Memorial Services, Aging Gracefully Sessions, Grief Support Groups, Bible Study, and other spiritual services, which are rendered timely and thoroughly documented in the electronic health record.
  • Facilitate weekly Spiritual Wellness discussions based on quotations, sacred writings, spiritual coping skills, contemporary issues, popular music, various religious holidays, current events, and participant interests.
  • Conduct Bereavement Assessments for caregivers and their families and provide grief support through condolence calls, Grief Support Groups, mailings, and Memorial Services.
  • Attend Interdisciplinary Team meetings and discuss the spiritual/bereavement needs of participants and their families
  • Plans and provides staff development on topics related to spiritual care.
  • Attend weekly huddles led by Spiritual Care Coordinator.
  • Adhere to all PACE Southeast Michigan standards, policies, and procedures.
  • Performs other related duties, as assigned by the Spiritual Care Coordinator.

QUALIFICATIONS (knowledge, skills, abilities):


  • Requires a high school education or equivalent.
  • Demonstrated knowledge of and experience providing spiritual care services; preferably in health care.
  • Positive, flexible, and solution-focused attitude with ability to communicate and collaborate effectively in a team setting.
  • Cultural competence and an ability to care for a diverse population.
  • Knowledge of and compliance with applicable local, state, and federal laws and regulations.
  • Ability to learn and use electronic health record system.
  • Proficient in Microsoft Word, Excel, Outlook, PowerPoint.
  • Must be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Must pass post-offer, pre-employment background check and health screening, including drug test, TB and MMR.
