Position Summary:
This position is part of the Work Ready Program Team. The Social Worker-LSW is responsible for assessing each participant's behavioral health status, developing and implemeningt a comprehensive plan of care, and providing individual, family, and group counseling, while maintaining all required clinical documentation in compliance with program requirements.
Work Ready provides intensive case management, supportive services, barrier remediation, job readiness, on-the-job training, job placement, job retention and educational enhancements to participants and their family members to promote career advancement and income improvement.
Primary Duties and Responsibilities:
1. Assess each participant and develop a behavioral health summary to develop a comprehensive plan of care based on individual and family goals, needs, interests, supports, barriers, limitations, and other health considerations
2. Conduct participant outreach to improve program compliance, provide counseling services and information to participants, and provide supportive services, as needed
3. Provide individual, family, and/or group counseling from a trauma-informed, evidence-based focus, including the implementation of short and long-term behavioral health interventions, including but not limited to drug and alcohol prevention and emotional support
4. Maintain all case documentation forms in a timely and accurate manner
5. Coordinate all services, activities, and supports, including agency referrals, to address and stabilize individual and family barriers pertaining to behavioral health
6. Coordinate with the participant and their family members to create a full family service plan to address identified needs in the household
7. Provide monthly behavioral health resources and on-demand services for participants and their family members
8. Review each participant's individual employment plan (IEP) for accuracy and compliance
9. Provide psychoeducational instruction and curriculum in a classroom environment adapted for different learning styles
10. Prepare and complete accurate case notes, written correspondence, reports, and other written materials in CWDS and HMIS
11. Design and facilitate activities and tasks to alleviate individual and family barriers to self-sufficiency and employment
12. Evaluate and measure participant outcomes and implementation standards based on classroom activities and the full service plan
13. Coordinate and participate in ongoing case reviews with agency staff to ensure all activities are directed toward the participant needs and interests
14. Identify integrated care opportunities and coordinate behavioral health services with primary care and/or health treatments
15. Participate in staff and supervisory meetings, trainings, and other agency events
16. Other duties as assigned

Qualifications:
This position requires a minimum of a Master’s Degree in Social Work from an accredited college or university and a PA Social Work license. Requires a minimum of two years’ experience in the human services field. Experience in working in employment/training programs from a trauma-focused, strength-based, and family-centered approach is preferred.
The person in this position must demonstrate outstanding organizational skills, and the ability to manage multiple priorities and concerns. This position requires an individual with strong assessment skills. This position requires an individual with knowledge of the Delaware County Social Service infrastructure and the ability to maintain effective working relationships with those service providers. The individual must have excellent communication skills, both oral and written, combined with strong interpersonal skills. The ability to work well with people from different cultural and socio-economic backgrounds is crucial to this position.
In addition, this position requires a licensed and insured driver with his/her own vehicle.
Job Type: Part-time
Pay: $22.00 - $28.00 per hour
