Social Service Assistant Job at Polaris Health & Wellness of Liberty
Our facility is curently seeking a Social Service Assistant to work closely with our Social Service Director. You must either have or will be able to complete the Online Social Service Designee course.
Responsible to assist Social Service Director in providing medically related social services so that each resident/resident may attain or maintain the highest practicable level of physical, mental, and psychosocial well-being. Promotes a climate, policies and routines that enable residents to maximize their individuality, independence and dignity. Services will be provided in accordance with Federal, State and Local regulations and governing agencies.
You will participate in discharge planning, interviews of residents and/or family members, setting up transportation to and from appointments, perform charting as directed by Director, assist in resolution of grievances and participate in the overall quality assessment for our residents.
We are looking for a team player to be an asset to our Team!!
Qualifications
- Social Service Certificate, 36-Hour Social Service Designee Certificate, preferred.
- One year of supervised social service experience in a healthcare setting working directly with geriatric individuals.
- Minimum of 2 years’ experience in a Skilled Nursing Facility preferred.
