HealthBridge provides chronic disease care services to patients in their homes. We work with patients and their care team (providers, caregivers, and healthcare services), through monitoring, counseling, advocacy and risk mitigation to fully engage them in their plan of care. Stabilizing chronic conditions leads to a better quality of life and a reduction of unnecessary visits to the Hospital or Emergency Department.

HealthBridge is seeking Qualified Health Personnel to become remote patient monitoring (RPM) Data Monitors with our team! You will be analyzing electronic health data with the supervision of an RN, NP, or MD. This role is integral to helping patients understand their care plan. You will be their healthcare advocate by checking on their status, reviewing medications, communicating with their providers, and helping to identify other issues that could negatively impact the patient’s health. It is critical that the Monitors develop a trusting relationship with the patient that encourages future engagement and compliance.

Other Key Responsibilities:

  • Follow a standard process for communication
  • Review health data with the patient, caregivers and providers
  • Recognize abnormal health data and share it with your clinical team to determine the best action to take based on established protocols
  • Ensure patient compliance with remote patient monitoring device

​Qualifications:

  • Licensed Practical Nurse (LPN) or Medical Assistant (MA)
  • Experience assessing patients with chronic disease and targeting risks
  • Strong organizational skills
  • Independent; self-motivated
  • Flexible
  • Comfortable with digital documentation and communication
  • Able to participate as part of a team, with each member leveraging their training and expertise, to provide the best service to our patients

Job Type: Full-time

Pay: $18.00 - $25.00 per hour
