Job Summary and Qualifications
The Case Manager shall be responsible for monitoring the patient’s plan of care as developed by the physician to insure appropriate services are accomplished in a timely fashion. The case management process is initiated prior to admission when possible, and extends across the continuum of care until the patient no longer requires management. The intensity of services provided is dependent upon the patient’s needs and the resources available to produce the desired outcome. The case manager works in concert with the physician and staff to ease the patient’s transition through the continuum and optimize resource utilization. The Case Manager coordinates the interdisciplinary team approach in assessing the need for discharge planning/case management. Also is a resource to staff members on community services and resource management. Case Manager should be able to work independently with minimal supervision. Case Managers report to the Director/Assistant Director of Resource Management.
What you will do in this role:
Review medical records for Medicare patients daily to determine whether the patient meets criteria for admission and continued stay as established by Interqual.- Evaluates and assess observation patients for appropriateness in observation status.
- During the initial assessment patients are screened for case management opportunities and discharge needs.
- Provides patient/family health education that supports recovery and enables the patient to be involved in decisions about their own care.
- Consults directly with physicians to determine their plan for care for the patient.
- Accompany the physician on rounds when possible; discusses alternatives to level of care as needed.
- Act as a liaison through effective and professional communication between and with physicians, patients/families/ hospital staff, and outside agencies.
- Assumes a leadership role with the interdisciplinary team to manage care through criteria driven processes, for the appropriate level of care, patient status and resource utilization.
- Conducts interdisciplinary team meetings to provide a mechanism for all disciplines to collaborate, plan, implement, and assist the plan of care; patient selection should be criteria based and interventions will be documented.
- Demonstrates knowledge of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives; monitors self-compliance and implements process changes to ensure compliance to such regulations and quality initiatives as it relates to the provision of Case Management services.
- Educates the unit staff on the principles of resource management, discharge planning, and case management.
- Plans developed during initial assessment or patient care conferences are documented in the patient’s record and communicated to personnel on all shifts.
- Monitors resource utilization “triggers” and intervenes to prevent over or under utilization of resources. Quantifies interventions as measures of program success; i.e. decrease in length of stay, avoidable days, and readmissions.
- Makes appropriate referrals to third party payer disease and case management programs for recurring patients and patients with chronic disease states.
- Achieves individual goals related to job performance.
- Able to demonstrate the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the following age groups: adolescent, adult, geriatric.
- Documentation is kept on all education and required training
What qualifications you will need:
- Registered Nurse, or Registered Nurse Diploma
- Current license to practice as a Registered Nurse in the state of Louisiana
- Five years nursing experience in an acute care setting; case management experience preferred.
- Certification in Case Management, Nursing, or Utilization Review preferred.
