Job Summary:
The Dietitian will identify and assess the nutrition needs of assigned residents and initiate interventions to achieve and maintain his or her nutritional status and enhance quality of life consistent with the patient/resident's goals. Assure compliance with standards of practice and regulatory requirements.
Essential Functions:
- Develops or oversee development of a complete nutritional assessment within the time frame specified by the facility or within 14 days of admission (whichever is shorter).
- Completes or oversees completion of all nutrition related MDS required paperwork as defined by facility policy.
- Completes quarterly as well as annual nutrition assessments and re-assessments upon change of status, as the need arises.
- Development and maintenance of ongoing tracking system for the identification and follow up of identified high risk residents.
- Identifies and assesses the nutrition needs of each resident and initiates interventions to achieve and maintain his/her nutritional status consistent with his/her goals.
- Effectively utilizes resident/family interviews to obtain pertinent information for assessment.
- Reviews the medical records and thoroughly incorporates appropriate information into the nutrition assessment.
- Development and maintenance of a communication system with the interdisciplinary team to identify and coordinate nutrition interventions.
- Effectively identifies and makes appropriate referrals for drug-nutrient interventions, social service interventions and mental health interventions.
- Effectively identifies and utilizes and/or makes appropriate referrals for nutrition support interventions.
- Develops an interdisciplinary plan of care incorporating nutritional needs and choices based on the resident's current goals and previous life-styles.
- Facilitates the resident's right to accept or refuse nutrition and hydration in accordance with the resident's advance directives.
- Maintains or ensures that information on the care plan is kept current and is consistent with documentation in the progress notes.
- Attends or oversees participation in required resident care conferences and documentation of follow-up information in the medical record.
- Conducts or oversees follow-up nutritional concerns within a time frame appropriate to the assessment and care plan.
- Conducts meals rounds/resident visitation on regular basis according to policy. Monitors and evaluates the nutritional status of residents in area of responsibility including satisfaction with food selection, preparation, service, and the overall dining experience.
- Evaluates and monitors the resident's eating ability and rehabilitation potential as related to the maintenance of adequate nutrition and make appropriate referral to OT/speech therapy.
- Ensures there is a system for documentation of results of resident follow-up.
- Plans and coordinates diet office functions to include accuracy and consistency of diet orders, nourishment/supplement distribution, and maintenance of resident dietary records.
- Participates in menu planning for regular and therapeutic diets in conjunction with FSD. Reviews and approves cycle menu changes, responds to requests from Resident Food Committee with menu revision and analysis.
- Effectively identify and perform nutrition education and counseling interventions.
- Instructs and counsel residents and their families on normal nutrition and therapeutic diets with consideration given to their education, readiness of learning, cultural and socio-economic background. Documents in medical record according to policy.
- Organizes and/or teaches group classes for residents and their family as needed.
- Is involved in or oversees development of a discharge plan for residents. Documents progress and discharge summary information as outlined by policy.
- Acts as a resource on nutrition for all staff. Provides direction to physicians, licensed Nursing personnel, Director of Nursing, and Administrator in nutritional care and service via ongoing communication.
- Is involved with planning and presentation of in-services provided to Foodservice and Nursing in relation to nutrition/Foodservice.
- Actively participates in the development, annual review and revision of Continuous Quality Improvement program.
- Updates and maintains pertinent policies and procedures as they relate to the therapeutic care of the resident.
- Maintains ongoing communication with FSD to keep informed of current Foodservice operations and concerns and may assist in daily operations as needed.
- Monitors all components of dietary services for regulatory compliance.
- Attends daily interdisciplinary meetings
- Maintains professional growth and development through seminars, conferences, and professional affiliations.
- High Risk charting for areas covered by Diet Tech.
- Assists in providing diet education for Culinary Staff and Nursing Assistants as needed.
EDUCATION
- Bachelor's or Master's Degree from an accredited nutrition or dietetic program plan IV or plan V institution and Registration by the American Dietetic Association or meets ADA requirements for eligibility.
- Licensed/Certified per State requirement.
- Maintains continuing Education Credits as mandated by the American Dietetic Association of 75 credits every 5 years.
KNOWLEDGE, SKILLS, ABILITIES
- Ability to perform nutrition assessments and care plans incorporating developmental needs of an elderly population group, has knowledge of the nutritional implications of CVA's Dysphagia, fractures, surgery, wounds, diabetes, HTN, CVD, renal disease, etc.
- Specific knowledge in or ability to make referral to or confer with Dietitian's and/or physicians specialized in renal diet restrictions, eating disorders, TPN, and other appropriate specialized areas.
- Basic understanding of Foodservice operations.
- Knowledge of OBRA regulations including comprehensive assessments, care planning, and resident rights.
PHYSICAL DEMANDS
- Ability to move from kitchen to resident care areas to conference areas within a reasonable amount of time to meet requirements for the quantity and quality of work.
- Ability to read, write, speak and understand English.
- Ability to endure repetitive periods of writing and/or typing at a computer terminal.
- Ability to hear and verbally communicate on a telephone and to hard of hearing residents.
- Ability to sit for duration of conferences.
- Ability to read written material at arms length (trayline) and 2 feet from a computer monitor.
- Ability to participate in more physical demands as directed by FSD. These items include special function support, month end inventory, receiving, etc.
