Join our team to be a part of the National Institute of Health’s historic HEALing Communities Study, awarded to the University of Kentucky, to reduce opioid overdose deaths by 40% in the next 3 years. Our recovery support program, HEALing Transitions, is a new line of service through Bluegrass Transitional Care to support individuals with Opioid Use Disorder, in partnership with the UK HEALing Communities Study. We need experts who are committed to helping individuals overcome barriers and get connected, or stay connected, to treatment. Our team is focused on reducing stigma associated with medication treatment for Opioid Use Disorder and seeing clients enter remission and remain in recovery.

About the role:
An employee in this position is responsible for providing care navigation services and support for clients and families in the UK HEALing Communities and Transitions’ programs. The nurse will conduct assessments, define treatment goals, and develop a care plan. Responsible for transition/navigation services in the community and home. The position utilizes the clinical navigation protocols for the Study and is responsible for working with leadership in data collection, management and evaluation.
Qualifications:
- Is a graduate of an accredited school of professional (registered) nurses, BSN preferred.
- Is currently licensed as a Registered Nurse through the State Board of Nursing.
- At least two year’s general nursing experience with addiction, home care, medical surgical, psychiatric, mental health care experience. Transition/Navigation nursing experience is preferred.
- Understands evidenced-based transitional/navigation services to prevent 30-day avoidable hospital readmissions, emergency room usage and improve patient outcomes.
- Complies with accepted professional standards and principles.
- Knowledge and basic understanding of the psychosocial and legal operatives in behavioral health patients and their families.
- Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team and research team.
- Understands all Bluegrass Care Navigators’ lines of service, hospice, transitions, palliative care, , adult day health care, PACE, home health, private duty and personal services.
- Must have excellent observation skills, good nursing judgement and communication skills. Ability to provide good written documentation in a timely manner.
- Demonstrates the ability to work with interdisciplinary team.
- Is self-directed with the ability to work with little supervision.
- Must have excellent collaboration skills in working with UK HEAL leadership and team.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Knowledge of disease processes, addiction medicine, symptom management, coaching process and community and health resources.
- Must be able to make home visits to patients/families in a variety of settings; mobile homes, apartments, homes, rehabilitation and clinic settings and condominiums that may not have elevators.
- Current Kentucky driver’s license and unlimited access to a reliable automobile.
- Ability to travel as needed to all transition sites.
- Ability to exercise judgement and discretion in confidential matters.
- Must have excellent written and oral communication skills. Should be comfortable in public speaking with varied audiences.
- Possess the ability to manage and interpret data.
- Experienced with providing effective education, feedback and direction with clients and families.
- Must have strong organizational and planning skills.
- Possesses and maintains good physical stamina and mental health. Has presented a pre-employment health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy.
- Possesses and maintains current CPR certification.
- Must be flexible and can manage multiple tasks and programs simultaneously.
