Post Acute Coordinator Job at US Acute Care Solutions
*Candidate must reside in Mountain Time Zone. Position will be remote with occasional site visits.
*MUST HAVE short term and long term rehabilitation experience.
The Post Acute Care Coordinator will work together with the Regional Director of Operations and staff as the primary person who is responsible for compliance of provider efficiencies. This position requires a self-starting, independent person who can work effectively with the team to facilitate accuracy in daily operations of our provider delivery process.
ESSENTIAL JOB FUNCTIONS:
- Builds patient profiles in Athena upon admission to the skilled nursing facility.
- Ensures patient profiles are correct and complete with demographic information.
- Prepares providers rounding schedules daily for each skilled nursing facility assigned.
- Schedules new admitted patients for provider at assigned facility location.
- Schedules follow-up and acute visits for newly admitted patients.
- Ensures regulatory visits are completed and rotated between physicians and APP monthly.
- Works with facility discharge planner to capture all patient discharges from facility.
- Runs bi-weekly audits to ensure all visits have been completed by provider.
- Communicates with skilled nursing facility medical records personnel to ensure visit notes are entered appropriately.
- Supports assigned clinicians in tasks needed.
- Tracks weekly encounters by providers.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Communication Proficiency.
- Excellent computer skills, including Microsoft Word and Excel skills.
- Ability to work with change.
- Strong organizational skills including documentation and file management.
- Attention to Detail.
- Exbibits effective written and verbal communication skills.
- Coordinates with company staff to accomplish program goals.
- Participates in planned meetings by post-acute care team.
- Communicates effectively with physicians.
- Ability to successfully manage numerous tasks.
EDUCATION AND EXPERIENCE:
- Bachelor’s degree or equivalent experience required
- 3+ years’ of healthcare experience
