Overview:
Masters degree in business, public health, nursing, integrative health, healthcare administration or other related field strongly preferred. Bachelor's degree required. Advanced training in project management, performance improvement and quality improvement methodologies is strongly preferred.
EXPERIENCE
At least 5 (five) years of experience working in a healthcare setting with progressively increasing responsibilities. Experience in data analysis, statistics and performance improvement is required. Experience managing populations and contract negotiations are preferred.

Office is based in New Haven but the ability to travel to locations across the health system from time to time is required. Occasional early morning or evening meetings may be required.

The Population Health Administrator will be responsible for the coordination, facilitation and execution of operational activities for assigned value based or population based programs that are implemented across the Yale New Haven Health System. 1. Operational oversight of assigned population based programs
1.1. Provide program management of implementation activities related to initiation of value based programs including development and execution of playbook to launch operational oversight of assigned programs
1.2. Support contract development and execute operational elements with the assistance of internal clients and clinically integrated network partners
1.3. Ensure readiness of necessary operating, data, and reporting capabilities by creating and tracking to detailed project plans
1.4. Develop marketing materials highlighting program performance and value proposition
2. Coordination of program requirements
2.1. Coordinate necessary resources to meet milestones of state, federal, or other grants engaged by YNHHS
2.2. Create detailed action plans aimed at improving key performance criteria in cost, quality and utilization for population(s) under management
2.3. Create and adhere to budget and resourcing requirements needed to execute program objectives
2.4. Maximize impact of the multidisciplinary clinical care coordination team being deployed to enhance the health of target populations
2.5. Ensure dissemination of requirements and updates to stakeholders and clinical practice sites
2.6. Compliance with and timely response to all programmatic reporting requirements
2.7. Coordinate development and dissemination of educational materials to raise awareness about the population(s) to target audiences
3. Establish performance improvement methodologies
3.1. Collaborate with information technology partners to create and align dashboards with target quality and financial metrics to track performance
3.2. Monitor, evaluate, and report performance trends to leadership and clinicians
3.3. Analyze programmatic data to identify opportunities for improvement and inform interventions; develop action plans to address identified areas
3.4. Track to contract requirements and develop reporting templates for progress monitoring
4. Collaboration with program and organizational stakeholders
4.1. Point person for internal and external participants to ensure effective program performance
4.2. Communicates and aids in decisions making with clinical, financial, and operational management
4.3. Coordinates and participates in task forces and committees to provide input and make recommendations for care transformation and outcomes improvement
