- Bachelor's, Master's or Doctorate of Science in Physical Therapy - Required
- Licensed as a Physical Therapist by the Oklahoma State Board of Medical Examiners or a Graduate PT eligible for Oklahoma licensure or Physical Therapy student in the final semester of graduate school - Required
- BLS - Required
- Oklahoma State License *Compact License Allowed*
