Patient Care Navigator Job at Crown Health
JOB SUMMARY
At Crown Health, a ConcertoCare practice, the Patient Care Navigator is responsible for effectively engaging patients and ensuring access to care. The Patient Care Navigator does this by administering and designing patient schedules, conducting proactive outreach to patients, scheduling visits, communicating effectively with patients and other stakeholders and other administrative tasks as assigned.
Responsibilities
- Administers provider schedules in electronic scheduling platform. Sets up provider scheduling based on scheduling parameters.
- Schedules face to face, virtual and telephonic visits to ensure patient access to care. Responds to requests for appointments and adjusts schedules as necessary to accommodate the needs of the patients.
- Conducts proactive outreach to enrolled patients to support patient panel management activities.
- Monitors and assigns provider patient caseloads at the direction of practice management leadership.
- Facilitates PCP change notification to communities, POAs and Guardians.
- Supports insurance/patient eligibility verification as needed/assigned.
- Supports patient and proxy portal set up in EHR.
- Assists patients, caregivers, and POAs with getting connected to virtual telehealth appointments.
- Communicates effectively with patients, caregivers, facilities and POAs. Maintains a high level of customer service and patient experience. Is responsive to internal and external communication.
- Documents all pertinent patient information and call activities in appropriate electronic platforms.
- Attends all scheduled meetings.
- Creative problem solving, critical thinking, and empathy skills are essential. Professional interaction, active and passive listening skills, and the ability to utilize computer- based resources in a highly effective manner to educate and provide accurate responses to customer inquiries is crucial for success in the role, along with the innate ability to be compassionate and empathetic.
QUALIFICATIONS
- High School diploma or GED required.
- 1 year experience working in a medical practice as a Medical Assistant or Medical Receptionist is preferred.
- Intermediate proficiency working in Microsoft Office
- Experience working with electronic health records and telephony systems with the ability to perform in a high volume fast paced medical office
- Working knowledge of HIPAA (Health Insurance Portability and Accountability), medical terminology, insurance authorizations, health benefits plans, and medical procedures.
- Excellent written and oral communication skills.
- Proven ability to work both independently and as a productive member of a team
Base Salary/ Wage Range $20.00 - $21.63/hour plus annual bonus potential. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
