Job Summary and scope of responsibilities:
The Patient Navigator Supervisor assists with creating, providing, and managing training and development for Patient Navigators throughout the organization. They provide peer supervision to Patient Navigator team members and assist these team members in developing the skills and knowledge needed to perform their current role and, when appropriate, to achieve new roles within the organization. The Patient Navigator Supervisor also works directly with patients as needed and engages in outreach efforts within the larger community with the goal of decreasing health inequities. They work closely with other leaders to drive the ongoing development of an organizational-level Patient Navigator Team development and training program.

Primary Responsibilities:
- Provide support and supervision for Patient Navigator Team.
- Analyze training and professional development gaps to determine training needs and learning objectives for Patient Navigators
- Design and develop training programs and materials to support Patient Navigator Team members and meet identified objectives in partnership with organization leaders
- Assist with team hiring
- Implement team training and onboarding in coordination with other leaders
- Assist in evaluating training outcomes and effectiveness, including quality and performance outcomes where relevant, and make changes based on feedback from team and leaders.
- Assist in monitoring program outcomes, including quality and performance outcomes where relevant as well as qualitative feedback from team and leaders, in collaboration with the quality manager to drive improvement as needed.
- Help drive career development pipelines for the Patient Navigator Team with an equity, diversity, and inclusion lens
- Establish and maintain relationships with internal Neighborcare departments and external referral sources, community partners, and key stakeholders, including organizations where Housing and Street Outreach staff provide regular on-site services
- Coordinate continually with clinical operations and quality leadership to maintain program alignment with strategic priorities
- Collaborate with clinical leaders to sustain and improve professional activities and structures. Identify gaps in knowledge, skills, or practice within the Patient Navigator Team they supervise and work with clinical leaders to create and implement plans to address.
- Provide direct patient support as a Peer Navigator as needed to support patient access

Education Required:
- CHW Training
- Supervision Training
Education Preferred:
- Specialized training dependent on patient need
Certification Preferred:
- Professional certification relevant to dept and population (ex. HCAs Certified Peer Counselor)
Years of relevant experience required:
- 2 years working at Neighborcare
- 1 year working as a Lay Peer Navigator
Years of relevant experience preferred:
- 1 year experience in community support and outreach

Compensation
The wage range for this position is minimum $28.86 up to $38.75 maximum.
Compensation will be determined based on years of relevant experience.
