Job Summary:

Under the supervision of the Program Lead, the Full Service Partnership (FSP) / Outpatient Client Services (OCS) Parent Partner works closely with the parents/caregiver to provide support, education, advocacy and participate as a member of the treatment team. The FSP / OCS Parent Partner provides field-based support services in the home, community, or other location convenient for the parent/caregiver, and is available 24/7 for crisis response. The FSP / OCS Parent Partner completes all required paperwork in accordance with Aviva, DMH, DCFS, and COA standards.

Essential Functions:

o Ability to build protective capacity with families; and Core Practice Indicators (engagement, teaming, assessment and understanding, long term view, planning, cultural competence, transitions, crisis management).

o Available 24/7, responsible for responding and supporting parents/caregivers during crisis situations

o Complete and submit all documentation and Case Records Reviews in accordance with agency and Department of Mental Health (DMH) regulations including, but not limited to progress notes, client functional evaluations, outcome measures, and treatment goals

o Provide Trauma Informed Care and educate parents/caregivers

o Participation and/or assistance with the Parent Support Group (e.g. planning curriculum, facilitating groups, engaging participants, encouraging parents/caregivers to participate, etc.)

o In collaboration with team members, create and maintain Crisis Safety Plan for families

o Assist parents in identifying needs, barriers and problem solves

o Link and advocate for resources in the community to address the family’s needs

o Assist family to have a voice in advocating for their needs.

o Model and coach family in areas such as communication skills, anger management, problem solving, stress management, appropriate discipline, developmental milestones/capabilities, and parenting skills.

o Assist families with contacting and following through with community resources.

o Interface with DCFS, DMH, court systems, juvenile hall, schools and other community liaisons.

o Responsiveness to the families’ identified needs.

o Building and strengthening the child and family’s support system.

o Development of Family-centered help-giver skills.

o Ability to connect with families of various backgrounds and engage caregivers so they are open to working with the treatment team.

o Shares the story of one's own journey in order to connect, empower, and give hope to parents.

o Adheres to a non-judgmental, strength-based model that builds on strengths and hope, while also being realistic and practical.

o Work closely and maintain weekly visit (or as indicated) with parents and caregivers to provide intensive and supportive services needed

o Collaborate and team with other professionals to plan and coordinate immediate services to stabilize crisis situations

o Works collaboratively and cooperatively within a multidisciplinary team

o Assess the need to request flex funds and communicate need to immediate supervisor

o Consult on regular basis with supervisor and team members about pertinent issues regarding the family’s participation and treatment plan

o Assist parents/caregivers in keeping appointments, access and link parents/caregivers to court hearings, and other community or county programs.

o Sensitive and responsive to the cultural differences among the agency’s client population

o Participation in all agency required meetings and trainings

o Actively participate in parent support groups/activities

o Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse

o Actively participate in the quality improvement project

The FSP / OCS Parent Partner will take on special projects or additional duties as assigned by the Program Lead/Supervisor.

Competency Requirements:

-Ability to speak, read, write and understand English

-Ability to work effectively as a member of a multi-disciplinary team

-Ability to successfully work with families of diverse ethnic and cultural backgrounds

-Must have excellent oral and written communication skills

-Must have excellent interpersonal skills

-Knowledge of social services system and community resources

-Skilled in relationship building and conflict mediation a plus

-Basic computer knowledge (Windows, Microsoft word, Outlook, Excel)

-Bilingual (Spanish) preferred

Education and Experience:

-High School diploma or equivalent required

-Currently or has been a primary caregiver (relative, parent, grandparent) of a child with SED

-Parent Partner must have been the parent or primary caregiver (Primary caregiver is defined as a parent (not foster parent) who has 24/7 legal responsibility for the child/youth who received services from one of the following County departments: Department of Mental Health, Probation Department, and/or Department of Child and Family services; or from Regional Center.

-At the time of hire, Parent Partners must not have an open probation case nor DCFS for at least one (1) year.

Certifications/Licenses:

-Valid CA driver’s license

-Valid automobile insurance

Additional Requirements:

-Tuberculosis clearance

-Department of Justice/Fingerprint clearance

-Satisfactory completion of health screening

-Proof of legal eligibility to work in the United States

-Satisfactory driving record

Physical Demands:

While performing the duties of a FSP/OCS Parent Partner, the employee is frequently required to walk, drive, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to sit, stand, and reach. The employee is rarely required to run, bend, kneel or lift objects weighing more than 30 pounds.

Work Environment:

The work environment is indoor and outdoors. The noise level varies from moderate to loud. The employee will conduct home visits.

Pay: $43,000.00 - $43,680.00 per year
