West County Health Centers Inc. is a Federally Qualified Health Center (FQHC) and a national leader in transforming community health centers to patient-focused centers of care. WCHC has a consistent record and reputation of providing the highest standard of care within a community health care setting. WCHC provides access to comprehensive health care services at all our multiple community health center locations throughout West Sonoma County.
In addition to providing critical primary medical care, dental care and behavioral health services, WCHC also serves as a resource for holistic health to include meditation, healthy cooking, stress reduction and other classes that support communities in living a healthy lifestyle.
This position is part-time 30.0 hrs. per week.
Responsibilities
- Reviews medical records, confers with medical and other providers as appropriate, and interviews clients regarding dietary habits and medical history.
- Consult visits with patients of WCHC with focus on nutrition as an approach to health, in collaboration with Medical Provider
- Evaluates nutritional and medical information and performs nutrition assessments.
- Completes individual nutritional care plan and counsels clients.
- Independently consult with pregnant patients on perinatal nutrition as part of the Comprehensive Perinatal Services Program (CPSP)
- Serves as a resource on nutrition to HIV Healthcare Network patients and staff (this is optional part of job, and will allow possibility of expansion to benefitted full time position).
- Evaluates, selects, and develops nutritional education materials; provides education to clients and staff of health centers and partner agencies.
- Participate in Care Team meetings to include daily huddles, monthly staff meetings and other related meetings when appropriate.
- Participate in quality improvement, quality assurance, and innovation activities to ensure optimal level of care delivery at West County Health Centers.
- Actively work with Care Team to optimize efficiency, team collaboration, and communication.
- Assist in the coordination of warm hand-offs or “Transfer of Trust” between Care Team, Behavioral Health Staff, or other support services during the office visit.
- Participate in activities related to addressing the social determinates of a patient’s health.
- Attend department and agency meetings and effectively manage communication provided by e-mail, memos, or training to comply with changes in workflows, procedures, or policies. Document care in the electronic medical record and appropriately complete necessary documentation of care provided.
- Maintain certification, maintain ongoing clinical competence through formal and informal Continuing Education activities.
Skills & Experience:
- 2-3 years of Registered Dietitian (R.D.) experience preferred.
- Experience in Medical Nutrition Therapy (MNT), preferably in a clinical or hospital setting.
- Must have excellent written and oral skills, strong organizational and analytical skills.
- Required to have versatility, flexibility and a willingness to work within constantly changing priorities with enthusiasm and positive attitude.
- Strong interpersonal skills and have the ability to understand and follow written/verbal instructions.
- Spanish speaking preferred.
- Demonstrate a positive customer service, patient centered approach at all times.
- Experienced in group education and/or group medical visits, or interest to learn and participate in group education/support
- Strong technical aptitude - Office 365
If also working with HIV population:
- Considerable knowledge of: the principles, techniques and practices of sound nutrition and dietary patters; the educational methods and skills used in training others in the principles of nutrition and the manner in which the principles are applied to various phases of health programs; the nutritional requirements of people living with HIV/AIDS.
- Working knowledge of: the analytical and statistical research methods used in nutritional investigation and dietary studies; modern office practices, administration and recordkeeping; current sociological and economic problems pertaining to community health nutrition.
- Ability to: apply methods and techniques used in nutrition investigation and dietary studies; plan, organize and conduct training programs on nutrition; establish and maintain rapport with the community, other professionals and other staff; prepare clear and concise reports; analyze situations effectively and implement an appropriate course of action.
- Cultural competency with the patient populations served in the HIV system of care.
Physical Requirements:
- Must be able to move freely about the office and carry light loads (up to 25 pounds)
- Prolonged sitting and occasional standing required.
- Ability to work with office equipment, including computers
