PURPOSE:
Provide administrative & Clinical support services to nurses and providers as part of the Primary Care Medical Home team. Contribute to the efficiency and effectiveness of individual patient visits as well as to improved population health.
ESSENTIAL FUNCTIONS:
- Support care coordination prior to patient visits by:
- Call patients who have fallen out of care to schedule appointments with providers and nurses
- Reviewing daily reports and patient electronic health record to identify gaps in care
- Perform prior authorization for medications
- Monitor medication fax line
- Answer phone calls from patients and disseminate them appropriately
- Working with the PCMH team to anticipate patient and provider needs prior to patient visit
- Partnering with the PCMH team and the provider to maintain consistent patient flow
- Medical reconciliations prior to appointments when applicable
SECONDARY FUNCTIONS:
- Assist providers and nurses during patient visits by greeting and accompanying patients to the clinical areas
- Completing the intake and timely documentation of the reason for visit, accurate vital signs, height and weight, pain assessment in the electronic health record and reviewing referral status, patient self-management goals and other tasks as directed

- Contributing to a smooth flow of patients by monitoring the provider schedule with approval.
- Communicating pertinent information to the provider, including status and/or results of referrals, hospitalization, diagnostic imaging, and labs
- Supporting medication reconciliation by reviewing the medications list with the patient to verify current medications
- Supporting population health by discussing, investigating, and addressing gaps in care with the patient and provider
- Helping the patient identify self-management goals and documenting those goals as appropriate
- Performing Point of Care Testing per protocol
- Assisting with procedures as appropriate incorporating Joint Commission National Patient Safety Goals
- Ensuring patient has copies of educational materials and next visit information
- Communicating to provider any pertinent social determinants of health that may need to be addressed
- Contributes to the efforts and outcomes of the PCMH team and supports Population Health efforts between visits by Maintaining required training for competency of CPR, HIPAA and Compliance, Waived testing, Infection Control, sterilization (if appropriate), and other topics as assigned.
- Performing sterilization of instruments, if trained and deemed competent to do so
- Participating in outbound calling as assigned or as necessary to confirm appointments or bring patients into care
- Maintain strict infection control standards in accordance with CHS’ protocols and universal precautions OSHA and CLIA standards
- Communicate with patients in their preferred language, using interpretive services as necessary, and interpreting for provider if proficiency in language has been established
- Perform all duties in accordance with CHS Core Values, Compliance Program, and other policies and procedures
- Participate in quality and process improvement efforts in the department and the organization as requested or as directed
- Complete training and professional development requirements in a timely manner
- Perform other duties as assigned or directed
KNOWLEDGE, SKILLS, and ABILITIES
Minimum educational qualifications required:
- High school diploma or equivalent
- Graduate of an accredited medical assisting or other clinical training program such as a nursing program.
- CPR certification at the healthcare provider level or eligible to obtain certification
- Proficiency in the use of Microsoft Office applications, including Word, Excel, and Outlook
COVID-19 Vaccination Requirement:
CHS requires its employees to be fully vaccinated against COVID-19, where allowable under the law, unless they are approved for a reasonable accommodation based on a medical condition or religious belief that prevents them from being vaccinated.
- If you are vaccinated, you are required to have received all COVID-19 shots necessary depending on the type of vaccination (1 dose for Janssen, 2 doses for Moderna or Pfizer) prior to your first day of employment. Preferred knowledge/experience required beyond minimum qualification:
- Three years of medical assisting experience, with prior experience in a clinic or hospital setting strongly preferred
- Experience working with socioeconomically diverse, multi-ethnic populations
- Written and oral proficiency in Spanish
- Written and oral proficiency in a language other than English

TYPICAL PHYSICAL DEMANDS:
- Frequent standing, walking, and bending.
- Requires lifting of up to 15 pounds.
- Visual acuity sufficient to perform frequent work on a computer screen and review printed reports and other materials.
- This role routinely uses standard office equipment such as computers, phones, photocopiers, and filing cabinets.
- Requires the ability to hear and to communicate orally with others.
TYPICAL WORKING CONDITIONS: This job operates in a professional office environment. Potential exposure to communicable diseases and other conditions in a health center environment.
