We are currently looking for

part time (10 hours a week) Nurse Practitioner for our East Bay and Pawtucket locations

part time (16 hours a week, Tuesday and Thursday) Nurse Practitioner for our Newport Mental Health location

MAJOR DUTIES AND RESPONSIBILITIES:

• Perform complete initial assessment on patients to determining diagnosis and develop a plan of care including medication prescription if indicated
• Provide timely documentation of each encounter with a patient including symptoms addressed, need for current treatment, and rationale for any changes in treatment
• Approval of admission into CODAC Medication Assisted Treatment Program
• Approval and determination of admission starting dose according to medical induction protocol
• Consultation with supervisors, clinicians, mid-levels, and nurses on patient issues
• See patients for admissions, annual physical exams, dose changes, methadone/Suboxone/ Vivitrol, patient dosing non-compliance, medical issues relating to addiction, and high-risk/complicated patients
• On-call for dosing orders for nurses as needed
• Sign orders for dosing, medication tapering requests, and take-home medication requests, and treatment plans
• Review laboratory reports, EKGs, and hospital records
• Comply with Federal Confidentiality Law to protect rights of the consumer
• Attend meetings as required
• Occasional on-call responsibility
• Other functions normally associated with the medical protocols of CODAC’s Policy and Procedure
• Perform other duties as required.

QUALIFICATIONS: Must be licensed by the State of Rhode Island and have received graduated from an accredited program. Must have an X-Waiver.
