Sun River Health is looking for a full time Nurse Manager to support our Queens Health Home and Church Avenue sites.

POSITION SUMMARY
Responsible for developing and coordinating case management activities, workflows and training across all health home sites in accordance with evidence-based practice and/or regulatory/payer requirements to achieve clinical outcomes. Provides ongoing consultation and assistance to case managers on managing difficult/clinically complex cases. This position requires travel between our Queens and Brookyln sites as needed, but will mainly be housed in Queens.
ESSENTIAL FUNCTIONS
- Ensures staff receives appropriate orientation and training to care management, care planning and care coordination workflows
- Ensures best practices and policies and procedures are up to date and consistent with Health Home requirements; provide training to staff, accordingly.
- Participates in current process review and development of new and/or revised clinical workflows, tools, processes, policies and procedures relating to complex case management responsibilities.
- Develops and implements use of educational materials and ongoing training plans to ensure staff have the necessary clinical information and resources needed to meet clinical objectives
- Facilitates educational trainings for clinical staff on issues related to health literacy, medication management and reconciliation, and health promotion.
- Closely monitors clinical care, transitions of care, gaps in care, follow up of high-risk and/or complex clinical scenarios, clinical outcome, and rapid coordination of services for identified clients. Leads case consultation around high-risk clinical issues.
- Collaborates with supervisors to guide and monitor staff placed on performance improvement plans for clinical workflows and documentation.
- Works collaboratively with management team to conduct clinical operations analysis, prepare reports and provide recommendations for performance improvement.
- Monitors care manager assignments and makes recommendations for adjustments based on staff experience, severity of cases and type of intervention(s) required.
- Monitors and evaluates patient care plans and services for quality and appropriateness through direct observation, documentation review, and conferences with staff
- Leads team case conferences and coordinates interdisciplinary care planning
- Makes field visits with care team members to promote transitional care and ensure effective care planning as needed.
- Maintains current knowledge of policies and procedures, state and local laws, industry best practice standards by participating in formal education programs, conference, workshops and professional organizations
- Participates in Agency evaluation program as part of the quality improvement team
- Abides by and promotes agency administrative policies
- Performs other related duties and projects, as assigned.
EDUCATION/EXPERIENCE:
- Bachelors of Science degree in Nursing, Master’s Degree preferred; or
- Registered Nurse currently licensed in the State of New York; or
- Two years supervisory or teaching experience in a clinical setting.
- Experience in managed care or home health care management highly preferred
- Strong computer skills (Word, Excel, Outlook, PowerPoint)
