We are searching for Medical Social Workers to perform diversified duties both within and in support of the social work functions.

Full Time and Part Time opportunities available.

Shift: day shift (8a-4:30p with some flexibility in start and end time). Every 5th weekend rotation.

Location: Charlton Memorial Hospital, Fall River MA

• Provide case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills.
• Identify and address psychosocial factors that may impede the patients’ ability to optimize benefit from the health care delivery experience.
• Intervene as necessary preventing over and under utilization of health care resources.
• Actively participate in discharge planning to assure timely individual patient discharges within the scope of position responsibilities.
• Responsibilities may include on call/shift coverage and holiday coverage.

$3000 new hire sign-on bonus with LICSW or LMHC license at time of hire. (rehires termed more than 1 year also eligible)

Education and/or Experience Requirements:

• Current Massachusetts Social Work license at a minimum of a LCSW is required.

Must be fully vaccinated against seasonal Influenza and COVID 19 and provide required vaccination documentation, unless approved for an exemption or deferral by Southcoast prior to starting employment.
