As an essential part of the Health Services team, establish and maintain accurate and detailed medical records, incidents reports and census reports for all residents in accordance with accepted standards. Perform a variety of clerical tasks involving the preparation, maintenance and processing of resident, unit and nursing records and related data for the unit. Additionally, provides back up assistance for Nursing Scheduler.
Responsibilities
- Establish and maintain a complete medical record from resident admission to discharge in accordance with professional standards and procedures for all areas of the healthcare continuum.
- Ensure that medical records are complete, properly organized and easily accessible to facilitate retrieval of information.
- Ensure proper coding (according to most recent standards) and maintain up-to-date information of each medical record.
- Maintains Payroll-Based Journal (PBJ) input and submission per Federal requirements.
- Develop and maintain a medical record sign-out/availability system and that all medical record requests are conducted per WCBR policy.
- Audit all new medical records within 72 hours of admission.
- Complete monthly audits for Quality Assurance.
- Back up to Nursing Scheduler for Payroll-Based Journal (PBJ) input and submission per Federal requirements.
Qualifications
- Demonstrated administrative skills including typing, word-processing and presentation skills, filing and note-taking ability.
- Demonstrated ability to schedule, coordinate, and implement activities with numerous details and multiple deadlines; ability to perform under pressure.
- Ability to maintain confidentiality.
- Knowledge of general office equipment including computer, fax, telephone, printer, shredder, copier.
Requirements
- Associate’s degree, RHIT preferred with one to three or more years of experience. Equivalent five years of work experience can be substituted for education.
- CPR and First Aid Certification required.
- ICD-10 Coding, preferred.
