Castle Senior Living is looking to hire a LPN - 2nd Shift Floor Supervisor. Castle strives to create the most satisfying living experience for the residents we serve and to be the most respected senior living company to work for by going above and beyond in any way possible.

Our ideal candidate will oversee the nursing services provided to the residents of our new facility during 2nd shift with assistance from the DON, ensuring they're in accordance with company policies and state regulations.
CSL is looking for two individuals to fill these roles sharing the every other weekend requirement.

Essential Duties and Responsibilities:
  • Assume self-responsibility for all direct patient care activities as indicated by the DON within the scope of the Nurse Practice Act, DHS 83 and with CSL policies and procedure
  • Possesses knowledge of nursing principles (geriatrics, senior living, and long-term care), professional standards of nursing practice and ability to apply to resident specific circumstances
  • Possesses knowledge of Wisconsin state regulations for CBRFs and/or RCACs
  • Provide direct patient care according to the ISP and physician orders
  • Provide appropriate information to assist the DON in the preparation and maintenance of an effective ISP
  • Document detailed notes in residents’ electronic medical records that reflect the therapeutic plan and specific aspects of the ISP
  • Administer prescribed medications to residents in accordance with physicians’ orders
  • Carry out duties in accordance with the facility’s mission, vision, and core values
  • Demonstrate honesty and integrity in the care and use of resident and facility property
  • Maintain a good rapport with families
  • Complete follow-ups on: MD notification or change of condition; Family notification of change of condition, labs, physician orders, etc.; Accidents/incidents; New admission chart review and clarifications; Discharge chart review
  • After follow-ups, update ISPs as warranted and perform physical evaluations related to change of condition
  • Oversee the 24-hour report. Make routine checks on any resident who is acutely ill or has any change in condition
  • Communicate with all departments to ensure quality care of residents
  • Maintain effective, positive working relationships
  • Oversee the care provided by caregivers/CNAs, as well as ensuring their documentation is accurate and complete. Counsel and discipline caregivers/CNAs as necessary
  • Attend and participate in weekly leadership meetings
  • Assist with OSHA compliance requirements for all employees
  • Conduct tours and take inquiries as needed

Education / Qualifications:
Degree from an accredited school of nursing; valid and current nursing license in the state of Wisconsin in good standing. One year of nursing experience working with seniors; experience within an assisted living, memory care or other long-term care environment preferred. Managerial experience preferred Must be willing to work 2nd shift 3pm-11pm hours and every-other weekend, and holiday rotations. Excellent interpersonal skills; must be able to connect with residents, families, and associates Strong customer service orientation to older adults; exhibit a caring and compassionate attitude. Ability to make responsible choices and decisions that are in residents' best interest. Excellent written and verbal communication skills; must be proficient in Microsoft; ECP software experience a plus. Ability to problem-solve and communicate complex issues; ability to work collaboratively with all co-workers.
