The Coordinated Homelessness Prevention Manager will work with other Journey Home staff and Coordinated Access Network (CAN) Partners to develop a coordinated homelessness prevention system guided by best practices and lived experience of housing instability in the past 5-7 years.

SUMMARY OF DUTIES/RESPONSIBILITIES:

  • Facilitate regular meetings of all the region’s homeless prevention providers and other staff who work with at risk households.
  • Work with other homeless prevention providers and partner agencies to establish and continually improve a standardized referral process for connecting households at risk of homelessness with prevention providers. Oversee referral process and troubleshoot any challenges as needed.
  • Work with other prevention providers to put a system in place that comprehensively covers the entire geographic region and prevents duplication of services to clients.
  • Engage persons with lived experience in the development and implementation of a coordinated homelessness prevention system.
  • Provide training to any prevention providers and partners in the region to orient them to the Coordinated Homelessness Prevention processes.
  • Manage and maintain a “By Name List” of all households actively seeking homelessness prevention services.
  • Serve as the representative of the prevention providers at local Coordinated Access Network (CAN) meetings advocating for the needs of the staff and at-risk clients.
  • Establish new relationships and new partnerships with organizations external to the CAN, such as healthcare, faith-based or community-based providers who may regularly interface with households at risk of homelessness.
  • With other CAN staff, provide training about the coordinated homelessness prevention system and homelessness crisis response system to internal and external CAN providers.

KNOWLEDGE, ABILITIES, QUALIFICATIONS:

  • Associate’s degree or sufficient level of experience within the field
  • Lived experience of homelessness or housing instability in the past 5-7 years required
  • Resident or former resident of zip codes 06120 or 06112 strongly preferred
  • Bilingual, English/Spanish preferred
  • Must be willing to work flexible hours including early morning, evening and weekend hours as required
  • Proven record assisting homeless clients and/or clients at risk of homelessness
  • Knowledge of federal and state funding housing programs preferred
  • Basic knowledge of Microsoft Office suite and data entry
  • Excellent organization skills and attention to detail required
  • Strong leadership and facilitation skills
  • Knowledge of CT HMIS preferred, but not required
  • Strong communication skills, including written correspondence and phone personality
  • Ability to work independently, adjusting to changing priorities
  • Ability to work with groups and balance multiple points of view
  • Maintain a valid CT Driver’s license, reliable transportation, and adequate auto insurance as required by CT state law.

Compensation & Benefits:

This is a full-time, exempt position

Salary range is $50,000 – $60,000; salary based on experience and qualifications, not salary history
