About us

Greenridge Post-Acute is a beautiful, recently remodeled 5-star facility located in El Sobrante, CA that provides skilled nursing for 60 beds and assisted living next door to our clients. We are currently looking for a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) to join our team.

Our LVN’s and RN's are critical in providing excellent patient care. They help with passing medication, identifying changes of condition, and enteral feeding, and much more.

Responsibilities and Duties:

We are seeking experienced and new grad nurses to support medically complex seniors in an effort to better manage their health.

Nurse care management: observe, plan, implement, coordinate, monitor, and evaluate the plan of care with the attending physicians.

Provide well-documented, accurate and sensitive in-person advice, health education and self-care instruction.

Support patients during times of crisis or transition to prevent avoidable hospital readmissions.

Reach out to patients identified as high-risk.

Qualifications

California LVN or RN License (Preferred):

Ability to thrive in a fast-paced environment and adapt to change.

Ability to work both independently and collaboratively within a team.

Demonstrated clinical assessment skills.
