Job Title: Field Licensed Practical Nurse (LPN) - Hospice

Location: Washington, DC 20001

Duration: 4 Weeks

Shift: 8 am – 5 pm

Job Summary:

We are seeking a compassionate and experienced Field Licensed Practical Nurse (LPN) to join our team in providing high-quality hospice care to patients in the Washington, DC territory. The ideal candidate will have at least one year of experience as a home health or hospice nurse, with experience caring for geriatric and terminal patients. The LPN will be responsible for providing direct patient care, medication administration, and symptom management.

Responsibilities:

• Provide skilled nursing care to hospice patients in their homes

• Administer medications and treatments as prescribed by the physician

• Monitor and document changes in patients' health status

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Educate patients and families on the management of symptoms and medications

• Maintain accurate and timely documentation of patient assessments and interventions

• Provide emotional support to patients and families during end-of-life care

• Mileage reimbursement between patients at .50 cents per mile

Qualifications:

• Active LPN license in Washington, DC (not a compact state)

• Minimum of 1 year of experience as a home health or hospice nurse

• Geriatric and terminal patient experience preferred

• EMR experience required

• Valid driver's license, access to dependable vehicle, and clean driving record required

• Strong communication and interpersonal skills

• Ability to work independently and as part of a team

• Willingness to work the 8 am – 5 pm shift, with no weekends on-call

• COVID vaccination plus booster required, with no exemptions accepted

• All overtime must be pre-approved
