POSITION SUMMARY:

EPIC Rehabilitation and Nursing at White Plains is a **BRAND NEW** Skilled Nursing & Rehabilitation center located in the heart of White Plains. This SNF is part of our " Epic Healthcare Management" group of facilities. The role of the Licensed Practical Nurse is to provide day to day support to the Unit Manager/Charge Nurse/Supervisor, respond appropriately to resident/family needs within the scope of practice and organizes and oversees the assigned unit activities and staff assignments
- Utilize the nursing process in the care of designated Residents.
- Assure Assessment/Data Collection Documents clinical data on all assigned Residents per regulations.
- Report findings of ongoing changes /concerns to the Charge Nurse, Nursing Supervisor or Nurse Manager.
- Deliver direct Resident care consistent with the policies and procedures of the Facility.
- Administer medications and treatments in accordance with policy and procedure.
- Deliver care incompliance with Resident’s Rights and federal/state regulations.
- Respond appropriately to Resident/Family needs within the scope of practice.
- Identify Resident/Family responses and notifies designate RN in timely manner. If appropriate, consult with other members of the health care team
- Ensure the safety of all Residents in accordance with Facility/Nursing policy and procedure.
- Prepare Resident assignments for Certified Nursing Assistants, considering the category of needs of the Resident and competency of personnel.
- Address and communicate lab results, test results to RN supervisor, unit manager and MD.
- Communicate Residents’ incidents and concerns to RN supervisor \ unit manager and MD.
- Demonstrate effective interpersonal skills necessary to ensure quality patient care.
- Demonstrate an awareness of Resident’s rights and reports any concerns.
- Assure CNA documentation records (ADL’s BM, Meal intake, etc) are completed. Communicate to Charge Nurse/Unit Manager and other member of the care team any pertinent resident findings.
- LPN may not flush Picc lines, subclavian lines, implant ports or change dressings or draw blood from any of these lines
- Must be a graduate of an approved school of Nursing.
- Licensed and currently registered in New York State as Licensed Practical Nurse.
- Previous experience with geriatric patients is desirable, not required.
- Must have a sincere desire to work with the elderly, chronically ill, and disabled.
- Pharmacological background in theory and supervised clinical experience with medications.
- Thorough knowledge of principles and methods involved in handling of sterile/clean materials and daily hygienic care of residents. Preventing spread of disease by proper disposal of infected material and cleaning of equipment.
- First aid assistance in emergencies, must utilize good judgment in emergency situations.
- Must have a thorough knowledge of medical, social, and psychological needs of patients.
- Must possess a spirit of cooperation, caring and enthusiasm in order to create an atmosphere conducive to rehabilitation, optimal health and growth.
