Job Summary:
The Licensed Nursing Assistant (LNA) is a Para-professional employee, trained to provide personal care and health related services in the home. He/She functions under the direction, instruction and supervision of RN/Clinical Manager and Administrator.
Educational and Professional
- Successful completion of competency testing which meets guidelines outlined in Federal Register. (Required)
- Successful completion of state approved Licensed Nursing Assistant program. (Required)
- Current licensure in the State of New Hampshire. (Required)
- Current CPR certification (Preferred)
- Valid driver’s license and/or auto insurance, when applicable.
- Must have reliable transportation and the ability to travel from place to place. Clean driving record in compliance with the State of New Hampshire.
Knowledge and Abilities
- Good oral, written and reading communication skills.
- Resourcefulness, flexibility and integrity.
- Ability to work independently.
- Proficient in taking vital signs.
- Ability to make meaningful observations and write brief and accurate
- reports when necessary.
- Respectful of people and their cultural, religious and ethnic differences.
Responsibilities:
- Assist client with personal hygiene.
- Plan and prepare diet appropriate meals as prescribed by the physician.
- Assists with activities of daily living, which include but are not limited to dressing, ambulation, toileting, and safe transfer techniques as directed by the Registered Nurse or Physical Therapist.
- Prompts client to take self-administered medications as per the care plan.
- Works with other community agencies involved with the client as directed by the Clinical Manager or Primary Nurse.
- Assists with treatments/therapies as ordered by physician and supervised by the Primary Nurse or Physical Therapist.
- Carries out assignments as instructed by LNA care plan and notifies Primary Nurse or Physical Therapist when unable to do so.
- Reports changes in clients’ personal condition and any changes related to physical surroundings to Clinical Manager or Primary Nurse or Physical Therapist.
- Documents care given at time of visit via EMR or on paper and submits weekly.
- Performs routine housekeeping duties to maintain a safe, clean and comfortable environment for the client.
- Transports client when necessary, as authorized by the Clinical Manager or Administrator or designee.
- Respects cultural and religious practices of the client and their families.
- Practices honesty, dependability, and maintains confidentiality in all client related matters
- Performs other duties as required by Administrator, or designee.
Physical Demands
- Lifting, carrying, pushing or pulling up to 50#.
- Patient transfers with no assistance for direct patient care.
- Standing, walking, stair climbing, stooping, kneeling and crouching while administering patient care in the home.
- Reaching and handling
- Adequate eyesight for reading, writing, driving and performing tasks.
Aptitudes
Have the ability to understand verbal and written instructions. Read and write documentation in patient record. Work with the hands and fingers to manipulate large and small objects.
